- 1. 0 PURPOSE EMERGENCY INSTRUCTION I-4.2 RECOVERY FROM SAFETY INJECTION 1.1 To delineate the steps required to term.l.nate Safety Injection and return the unit to a normal, shutdown alignment.
2.0 INITIAL CONDITIONS 2.1 The conditions as described in one of the following Emergency Instruction exist such that termination of Safety Injection is required or desirable. 2.1.1 EI I-4.0, "Safety Injec~ion Initiation" 2.1.2 EI I-4.4, "Loss of Coolant" 2.1.3 EI I-4.6, "Loss of Secondary Coolant" 2.1.4 EI I-4.7, "Stearn Generator Tube Rupture" 3*. 0 IMMEDIATE ACTIONS 3.1 None 4.0 SUBSEQUENT ACTIONS 4.1 The Senior Shift Supervisor/ Shift Supervisor shall verify that the initial conditions described in Step 2.1 above are.satisfied. The Operator shall then proceed as follows: 4.1.1 When RCS pressure is greater than 2000 open 1CV139, 1CV140 COMMENTS Once Safety Injection is reset it will not automatically reinitiate until after the Reactor Trip Breakers have been reset. CAUTION In order to prevent excessive loss of water to the eves Holdup Tanks do not operate with 1CV139, 140 open for a period longer than 15 minutes. Salem Unit' 1 1 Rev. 11
4.1.2 Refer to Table I. If any of the criteria listed under the column "Safety Injection Reinitiation Criteria" for the appropriate accident exist at any time after Safety Injection is reset, manually reinitiate Safety Injection. 4.1.3 Maintain Safety Injection in operation until the criteria listed in Table I under the column "Actions Required After Reinitiation" for the appropriate accident are satisfied. 4.2 Control Auxiliary Feedwater flow to maintain an actual level of approximately 33% in the Narrow Range in all unaffected Stearn Generators. 4.2.1 If Auxiliary Feedwater flow has been interrupted to any Steam Generator for more than 5 minutes and the level has coincidentally been less than 10% in the Narrow Range for more than 5 minutes, flow must be 4 maintained less than lOxlO LBM/HR. until the level is greater than 10%. 4.3 If the Safety Injection was due to a break inside the Containment, de-energize the following circuits: Salem Unit 1 2 I-4.2 Refer to Appendix 1 to determine indicated level which will assure approximately 33% actual level. This applies-throughout the recovery. Circuits may be re-energized at the direc.tion of the Shift Supervisor to perform specific evolutions after Safety Injection is reset. Rev. 11

4.8 If the Reactor Coolant Pumps are not runninq, start at least one pump IAW OP II-1.3.1, "RCP Operation", in an unaffected loop. 4.9 If normal charging and Letdown were established in step 4.7 then, open the Centrifugal Charging Pump Mini-Flo Valves, 1CV139 & 140 if closed then stop the flow thru the Boron Injection Tank by closing Inlet Valves 1SJ4 and 1SJ5. Stop the Centrifugal Charging Pumps which are not being used for normal charging. 4.10 If normal charging & letdown cannot be established, maintain pressurizer level greater than 20% by periodically cycling 1SJ4 or 5 and 1SJ12 or 13. - 4.11 STOP the RHR Pumps and Safety Inje*ction Pumps 4.12 Return the Service Water System to Normal Lineup IAW OP V-1.3.1, "Service Water - Normal Operation" 4.12.1 Reestablish Service Water to the Turbine Building Salem Unit 1

4.13 Return Diesel Generators lA, lB and lC to normal operation IAW OP IV-16. 3. 1, "Emergency Power - Diesel Operation" 4.14 Return the Containment Ventilation System to normal operation IAW OP II-16.3.1, "Containment Ventilation Operation". 4.15 Return the Auxiliary Building ,Ventilation System to normal operation IAW OP II-17.3.1, "Auxiliary Building Ventilation Operation." 4.16 Return th~ Control Room Ventilation System to normal operation IAW OP II-17.3.2, "Control Room Ventilation Operation." 4.17 Verify the Emergency Control Air Compressor stops when Control Air pressure is normal and return the Control Air System to normal operation IAW.OP V-5.3.1, "Control Air System Operation." 4.18 MAINTAIN Stearn Generator levels in accordance with OP III-10.3.1, "Auxiliary Feedwater System Operation." 4.18.1 Refill the Auxiliary Feedwater Storage Tank as necessary. 4.19 Restore power to 11-14GB4 if possible and return the Stearn Generator Drains and Blowdown System to normal operation IAW OP III-13.3.2, "Stea.rn Generator Blowdown - Normal Operation". 4.20 Return the Containment/Plant Vent Radiation Monitor to normal operation IAW OP IV-11.3.3, "Process Radiation Mqnitors Normal Operation. 4.21 Restore power to 1WL96 and 1WL97 if possible and return the Inlet Valves to the Gas Analyzer, from the Reactor Coolant Drain Tank and Pressurizer Relief Tank, to normal by opening the following valves: 1WL96 1WL97 1PR18 1PR17 4.22 Drain and refill Boron Injection Tank from the Boric Acid Tanks IAW OP II-4.3.5, "Flushing and Draining the Boron Injection Tank" I-4.2 1WL96 - lCCDC Ckt13 1WL97 - lBBDC Ckt34 DO NOT remove the power from 1SJ4, 5, 12, 13 as this will make the Boron Injection Tank inoperable. CAUTION DO NOT isolate the Boron Injection Tank during the refilling process or during the subsequent sampling to insure the flow path thru the BIT is operable. NOTE The BIT Boron Concentration is below the Tech Spec limit. 4.23 Return the Safety Injection System to normal IAW OP II-4.3.1, "Safety Injection System - Normal Operation." -4.24 Make a visual inspection within the Containment when entry is permissible. 4.25 After the plant has stabilized, sample the Reactor Coolant System for boron concentration and H2. Adjust boron as necessary IAW OP II-3.3.6, "Boron Concentration tontrol" for the desired plant conditions. Degas the RCS as necessary IAW OP II-3.3.11, "Volume Control Tank Degassification". 4.26 Calculate the Shutdown.,Margin IAW the Reactor Engineer's Manual to ensure that the Reactor is shutdown by greater than 1.6% Delta k/k. Salem Unit 1 8 RWST and BAT levels may be low Rev. 11
4.27 Perform the following reviews of the Reactor Trip/Safety Injection: 4.27.1 INITIATE AD-16, "Post Reactor Trip/ Safety Injection Review and Unit Startup Approval Requirements." 4.27.2 Complete Attachment No. 1 "Post Safety Injection Data" and attach it to AD-16, "Post Reactor Trip/Safety Injection Review." 4.27.3 INITIATE an Operational Incident Report in accordance with AP-6, "Incident Report and Reportable Occurrence Program." 4.28 If, at this time, it becomes neces.sary, take the plant to the Cold Shutdown condition IAW IOP-6, "Hot Standby to Cold Shutdown." NOTE I-4.2 Do not continue until requirements of AD-16 "Post Reactor Trip/Safety Injection Review" are completed and approved. 4.29 Upon completion of AD-16 "Reactor Trip/Safety Injection Review" Sect. 4. 0, "Approval Requirements for Reactor Startup" withdraw the Shutdown Banks as follows: 4.29.l Reset the flux rate t+ip by momentarily taking the RATE MODE switches, on each NIS POWER RANGE A drawer, to the RESET position. Salem Unit 1 9 Any reactor trip from power will generate a negative rate trip signal which seals in and must be manually reset. Rev. 11
4.29.2 Depress the CLOSE pushbutton on the control console, for REACTOR TRIP BKR A, verifying the breaker does close. 4.29.3 Depress the CLOSE pushbutton on the control console for REACTOR TRIP BKR B, verifying the breaker does close. \\ 4.~9~4 Depress the STARTUP pushbutton, on the Control Console, and verify each Shutdown and Control Rod Step Counters reset to zero.
- 4.29.5 Commence withdrawing Shutdown Bank A,B, C and D, in that order, Salem Unit 1 to their fully withdrawn position.
10 I-4.2 If the STARTUP pushbutton has been pushed prior to closing the Reactor Trip Beakers, a Rod Urgent Failure Alarm will be present. This can be cleared by pu~hi'ng either the STARTUP pushbutton or the ALARM RESET pushbutton. Rev. 11
4.30 With Stearn Generator levels within their normal operating bands and just prior to commencing startup, perform the following: 4.30.1 Place the Stearn . Generator Feedwater Controls in MANUAL and run the valve demand to zero. 4.30.2 Reset the Feedwater Isolation signal by depressing the Train "A" and Train "B" FEEDWATER ISOLATION RESET pushbuttons on the control console. 4.31 Return to Power.Operation IAW IOP_<:'.'3, "Hbt Standby to Minimum Load", and IOP-4, "Power Operation." I-4.2 APPENDIX 1 DETERMINATION OF ACTUAL PRESSURIZER AND STEAM GENERATOR LEVELS To determine actual Pressurizer or Steam Generator Levels, utilize the attached Tables as described below. Tables 1-5 are to be used for the Steam Generators Tables 6-11 are to be used for the Pressurizer

Obtain the Average Containment Temperature from Computer Point Ul304.
- 2.
Refer to the appropriate Table for the pressure in the Steam Generator and Pressurizer.
- 3.
Locate the desired Actual Level under the column for the Average Containment Temperatures.
- 4.
From the column headed "Indicated Level (%)"determine the indicated level which should be maintained to ensure the Actual Level is maintained at or above the Desired Value. NOTE If the computer is unavailable or the containment average temperature is considered to be unreliable, maintain the indicated level 35% above the desired level.
