1.0 Purpose

1.1 To provide general guidelines for use by Performance Department I&C Personnel when maintenance is to be performed on installed plant equipment.

TITLE GENERAL PROCEDURE FOR TROUBLESHOOTING, REWORK AND REPAIR OF INSTALLED PLANT EQUIPMENT

3.0 Precautions

3.1 Insure all necessary tagging has been completed prior to starting work.

3.2 Obtain permission to perform the work from the Senior Shift Supervisor/Shift Supervisor prior to starting work if required by the work order *

  • 3.3 All necessary precautions shall be taken to insure that personnel radiation exposure is kept as low as practical.

3.4 Any.temporary jumpers which must be installed to troubleshoot shall be installed in accordance with Section 3.4.2 of the Performance Department Manual.

3.5 When troubleshooting instrument channels which have process control functions, precautions shall be taken

  • to prevent inadvertent actuation or disabling of Control Systems and/or devices. The Shift Supervisor/Control Operator shall be kept informed of any changes in channel integrity that will affect control of a process.

3.6 Insure all test equipment used for troubleshooting,

  • rework or repair is currently calibrated if required by the individual test equipment procedures.

3.0 Precautions

-3.7 Any permanent wiring or connections which must be disturbed for troubleshooting, rework or repair shall be appropriately tagged so that they are properly restored upon.completion of work.

3.8 When troubleshooting any equipment which provides an

  • input to the *solid State Protection System, the appro-priate procedure for that channel must be used to insure that the channel is removed from service and to insure that an inadvertent trip signal is not generated during troubleshooting~ This procedure shall be speci-fied by the job supervisor on the work order before the integrity (lifting of leads, removal of PC boards or plugs, change in switch lineup, etc.) of the channel is disturbed*.

3.9 Any precautions unique to the particular job shall be specified in the Special Instructions section of the work order form *

4.0 Instructions

4~1 The work *order system as described in AP-9 shall be used. in conjunction with this procedure to describe the problem, to specify any special precautions and to document the actions taken to correct the problem. Each problem shall have a work order to cover the job.

Appropriate documentation as required by the Perf or-mance Department Manual will also be completed.

  • PD 14.1.001 REV 2 ON THE SPOT CHANGE 1 PAGE* 4A If the equipment on which work is to be perforfUed is specified as Safety Related, Operability of an alarm or reactor trip/safeguard function shall be verified by documenting proper operation of the component through use*

of a Functional, Calibration, Calibration Check, Instrument Calibration Data Card, Solenoid History Card or Pneumatic Valve and Positioner Data Card entry. In the case of items that require a retest by the Operations Department (i.e. valve timing, pump operability checks) this shall be documented in the followup/retest section of the workorder form (AP-9).

The followup/retest .required shall be specified by the reviewer of the work order when it is known prior to the start of the work. If the operability test requirement becomes apparent during the course of performing maintenance or trouble shooting it will be documented in the description of work performed block on the work order form. Notification to departments other than Operations Department shall be documented in the description of work performed (i.e. work order number to other deparment) .

4.9 Record the necessary spare parts information on the backside of the ICD card as specified in the Perfor-mance Department Manual and on the Work Order.

4.10 Inform the Senior Shift Supervisor/Shift Supervisor that the device has been returned to service if re-quired by the work order .
