CORRECTIVE ACTION PROGRAM RELATED TO REACTOR TRIP BREAKER FAILURES No. 1 UNIT SALEM GENERATING STATION DOCKET NO. 50-272 This letter supplements our letters of March 1 and 8, 1983 which provided documentation of our investigation and proposed corrective actions related to incidents on February 22 and 25, 1983.
On those occasion the Salem Unit 1 reactor trip breakers failed to open upon receipt of an automatic trip signal from the reactor protection system.
In both instances, the manual trip was used to shut down the unit.
Since our transmittal of March 8, 1983, additional meetings and telephone conversations between NRC and PSE&G have resulted in requests for clarification or new information in several areas..
The attached report describes the corrective actions which the Company has taken or is pursuing and responds to the issues related to the events of February 22 and 25, 1983.
As demonstrated in this report, there has been sufficient investigation and evaluation to determine the cause of the incidents and to identify the necessary corrective actions to permit the restart of Salem Unit 1.
