Major Plant Activities in 2002 P January 2002-Licensee determined that the Component Cooling Water (CCW) pumps are susceptible to deadheading concerns early in an accident scenario.

P May 2002-Licensee identified that one CCW heat exchanger (HX) had significant tube leakage.

- Both HX's tubes were sleaved.

- Planned replacement of both HX's during upcoming outage (April 2003).

P November 2002-Operations Training Accreditation.

- Licensee is placed on probation and undertakes efforts to upgrade operations training program.

- Did not meet requirements of 10 CFR App. R PThis finding was a violation of 10 CFR Part 50, Appendix R, Section III.G.3, for the failure to provide the auxiliary feedwater pump fire area with a fixed suppression system
