One White Finding originally identified in Second Quarter of 2002
All Performance Indicators were within the Green band
Supplemental Inspection (IP 95001) for White Finding
Reason: Licensee failed to have a fixed fire suppression system installed in certain fire zones
Inspection started in March 2003, completed in December 2003
White Finding remained open for more than four quarters and the site remained in the Regulatory Response Column.

All findings during this assessment period were of very low safety significance (Green).
Severity Level III Notice of Violation and Proposed Imposition of Civil Penalty involved failure to:
- Monitor contractor fitness for duty
- Evaluate risk involved in continued unescorted access of a potentially impaired individual
