§ One White Finding originally identified in Second Quarter of 2002

§ All Performance Indicators were within the Green band l Supplemental Inspection (IP 95001) for White Finding l Reason: Licensee failed to have a fixed fire suppression system installed in certain fire zones l Inspection started in March 2003, completed in December 2003 l White Finding remained open for more than four quarters and the site remained in the Regulatory Response Column.

Safety Significant Findings or PIs n All findings during this assessment period were of very low safety significance (Green).

n Severity Level III Notice of Violation and Proposed Imposition of Civil Penalty involved failure to:

  • Evaluate risk involved in continued unescorted access of a potentially impaired individual
