  • A public forum for discussion of the licensees performance
  • NRC will address the licensee performance issues identified in the annual assessment letter
  • Licensee will be given the opportunity to respond to the information in the letter and inform the NRC of new or existing programs to maintain or improve their performance

z White finding involving the failure to provide adequate design control of the auxiliary feedwater system to ensure that the pumps would have been protected from failure due to air intrusion during tornado or seismic events, or from failure during potential runout conditions Regulatory

16 Kewaunee Assessment Results Regulatory

Degraded Cornerstone Degraded Cornerstone Degraded Cornerstone 4th Quarter 3rd Quarter 2nd Quarter 1st Quarter A 95002 inspection was conducted, in two parts, as a result of these inspection findings. The first portion was conducted in May and the second portion in August of 2006.

In August the NRC concluded that adequate actions had been taken to permit these two findings to be closed.

18 Safety Significant Findings or PIs (Jan 1 - Dec 31, 2006) z During the 4th Quarter of 2006 the Performance Indicator for Unplanned Scrams per 7000 hours0.081 days <br />1.944 hours <br />0.0116 weeks <br />0.00266 months <br /> changed from Green to White z In accordance with Manual Chapter 0305, a supplemental inspection will be scheduled as a result of this change.

19 Kewaunee Assessment Results Substantive Cross-Cutting Issues

  • Problem Identification and Resolution (PI&R)

- Issue opened in Annual Assessment Letter for 2004

- Issue discussed in Mid-Cycle Assessment Letter for 2005

- 11 findings attributed to weaknesses in PI&R during this assessment period

- 6 of these 11 share a common aspect of inadequate problem evaluation This Substantive Cross-Cutting Issue remains open.

20 Kewaunee Assessment Results Substantive Cross-Cutting Issues

  • Human Performance

- Issue opened in Mid-Cycle Assessment Letter for 2006

- 11 findings attributed to weaknesses in Human Performance during this assessment period

- 7 of these 11 shared the common aspect of failing to follow procedures

- 4 of these 11 shared the common aspect of failing to provide complete, accurate, and up-to-date procedures This Substantive Cross-Cutting Issue remains open.

21 Kewaunee Inspection Activities (Jan 1 - Dec 31, 2006)

Findings during this assessment period at Kewaunee were dispositioned as follows:

22 Non-Cited violations 4 Findings with no associated violation 24 of the 26 findings were characterized as Green The remaining 2 were addressed under Traditional Enforcement and were dispositioned as Severity Level IV findings A refueling outage took place at Kewaunee between September 2 and October 26, 2006. Major activities included:

  • In April, a Special Inspection was conducted to review an event that involved a reactor trip which did not occur while turbine trip conditions were present.

  • In May, a Special Inspection was conducted to review an event associated with control room activities during reactor start-up.

24 Kewaunee Annual Assessment Summary January 1 - December 31, 2006 z Dominion operated Kewaunee in a manner that preserved public health and safety z All cornerstone objectives were met with moderate degradation in safety performance z NRC plans to conduct baseline inspections at Kewaunee for the remainder of the assessment period z A 95001 inspection associated with the White Performance Indicator in Initiating Events Cornerstone will also be scheduled
