SUMMARY

REPORT During 2R13, PSEG, MQS, and FRA-ANP personnel performed ISI/NDE of selected Class 1,Class 2, Class 3, Class MC, and Class CC components of Salem Unit 2. These examinations constituted the thirteenth (13) (2nd Interval 3rd Period, 2nd Outage 2-3-2) refueling outage at Salem Unit 2, marking the end of the Second 10-year Inspection Interval of operation. Completion of refueling outage 2R13 ISIINDE exams concluded Salem Unit 2 Second Ten Year laservice Inspection Interval. PSEG properly satisfied ASME Xl Code allocation requirements. Allocation tables are contained in Enclosures 5 and 6.

This report is presented in 6 volumes as follows:

A. Applicable Documents ISI/NDE exams were conducted in accordance with the following documents:

Section XI of the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel (B&PV) Code, "Rules for Inservice Inspection of Nuclear Power Plant Components", 1986 Edition, no Addenda. Appendix VIII exams were conducted in accordance with 1995 Edition, 1996 Addenda, as amended by Final Rule. IWE/IWL exams were performed in accordance with 1998 Edition, 1998 Addenda, as amended by the NRC approved Safety Evaluation Report (SER).

ISI Section Xi Volumetric & Surface Examinations performed by FTI, and MQS Framatome-ANP and MQS under the direction of the PSEG ISI Group, conducted seventy-eight (78) manual ultrasonic (UT), thirteen (13) liquid penetrant (PT), four (4) Magnetic Particle (MT) Examinations and one (1) Radiography (RT) examination.

During 2R13, PSEG implemented Risk Informed Inservice Inspection (RI-ISI) of Nuclear Class 1 and 2 Piping Welds, formerly known as ASME Section Xl Exam Category B-F, B-J, C-F-1 and C-F-2. The bases of the Salem Unit 2 RI-ISI Program are documented in the Salem Nuclear Generating Station - Final Report

There were no other unacceptable exam results identified.

C. ISI Section Xl Reactor Vessel Visual (iWI) Examinations performed.

There were no IWI Examinations performed this outage D. ISI Section Xl Visual Examinations on valves and bolting PSEG's ISI Group conducted Three (3)Class 1 bolting exams and no Class 1 valve internal exams.

The following Items were examined:

Flange 2-PZR-MB-1-16 (Sum# 012500) A VT-1 was performed on the Manway Bolting.

Flange 2PS3 Valve (Sum# 255850) A VT-1 was performed on Flange Bolting.

ISI Section Xl examinations were conducted on the following systems:

- Reactor Coolant There were no unacceptable exam results identified.

E. ISI Section Xi Visual Examination of components supports (Code Case N-491)

PSEG Nuclear's ISI Group conducted 16 scheduled VT-1 and VT-3 visual examinations.

There were no unacceptable indications identified.

F ISI section Xl Visual Examination of Containment Integrity PSEG's Nuclear ISI Group conducted 166 VT-G IWE Visual Examinations.

There were no unacceptable exam results identified.

G. ISI Section Xl System Leakage Examinations The PSEG ISI Group conducted seventy-one (71) System Pressure Tests of System Functional or Inservice Tests, on Nuclear Class 1, 2, and 3, components in accordance with ASME Section XI.

A system leakage exam was performed in accordance with ASME Section Xl on all Class 1 Systems. The system leakage test was performed with the Reactor Coolant System in Mode 3 (normal operating temperature and pressure) with the insulation installed. No pressure boundary leakage was detected.

Work requests were initiated to correct minor mechanical leakage that was not corrected during the system walkdowns.

H. RCP Flywhee! Examination per USNRC Regulatory Guide 1.14 Framatome, under direct supervision from PSEG Nuclear's ISI Group, conducted period required ultrasonic exam-nations or, all four reactor coolant pump flywheels.

There were no unacceptable exam results identified.

1. Augmented Volu-metric Examinations of High Energy Piping Welds per Risk Informed Break Exclusion Region (RI-BER) Branch Technical Position MEB 3-1 During 2R13, PSEG Nuclear implemented Risk Informed Break Exclusion Region (RI-BER) of Nuclear Class 1and 2 Piping Welds, formerly known as MEB-3-1 Program. The bases of the Salem Unit 2 RI-BER Program are documented in the Salem Nuclear Generating Station - Final Report - Risk Informed Inservice Inspection Application (VTD-326046).

There was one (1) weld examined, to comply with the Risk Informed Break Exclusion Region (RI-BER).

There were no unacceptable exam results identified.

K. Augmented Bolted Connection Examr- per Code Case N 533-1 PSEG's ISI Group conducted eighty seven (87) Class 1 bolted connection V--2's in accordance with the ISI Long T erm Piarn Program and Code Case N 533-1.

There were no other unacceptable exam results identified.

0. Erosion I Corrosion UT Thickness Program In response to NRC Bulletin 87-01 and Engineering Directive MEC-94-1071, one hundred and forty nine (149) components were scheduled for UT thickness examinations.

As a result of these repairs / replacements, PSEG Engineering judgement, and procedural requirements for readings less than 70% of nominal wall, an examination expansion of sixteen (16) components was performed. This increased the total number of examinations to one hundred sixty five (165) components. The following is a break down of these examinations.

a. One (1)component was replaced.

b. One hundred and sixty four (164) were acceptable for continued service, based on minimum wall design requirements.

c. No components required a weld build-up.

There were four (4)Steam Generator Feedwater Elbows and Pup pieces replaced in Containment, three (3)inside the Bio-Shield and one (1) inthe Annulus. These were replaced as a result of low readings obtained during a previous outage.

There were no other unacceptable exam results identified.

P. Functional Testing of Selected Hydraulic and Mechanical Snubbers per Technical Specification 4.7.9 (c)

HYDRAULIC SNUBBERS (FIRST TEST SAMPLE)

Hydraulic snubbers were tested using the Technical Specification 4.7.9(c), 10%

sample plan by Wyle Laboratories using the API in-place snubber test machine and the Wyle Laboratories model 150-snubber test machine as follows:

- Two (2) S/G 1,000 Kip Rexnords

- One (1) MSIV 200 Kip Paul Monroe There were no degraded Snubbers, and no failures.

MECHANICAL SNUBBERS (FIRST TEST SAMPLE)

Mechanical snubbers were tested using the Technical Specification 4.7.9(c), 10%

sample plan using the Wyle Laboratcries model 150-snubber test machine as follows:

- One (1) Lisega small snubber

- Twelve (12) mechanical medium (PSA-1, 3 and 10)

- Iwo (2) mechanical small (PSA-1/4 and 1/2) 100% swap out of all PSA Mechanical and all Paul Monroe 200 KIP snubbers to Lisega type was completed during 2R 13. Each snubber was checked for freedom of motion and position verified. A! mechanical snubbers functional tested satisfactory.

A feedwater mechanical medium snubber found damaged and replaced in kind during 2R12 Was v'sual examired before and after shutdown.
