Unit 2 Docket No. STN 50-499 Response to Notice of Violation 499/8902-01 Houston Lighting & Power Company (HIAP) has reviewed Notice of Violation 499/8902-01 dated April 25, 1989. H14P concurs that the cited violation occurred. The violation has been fully addressed as the subject of the attached Licensee Event Report 89-002, " Failure to Properly Restore Control Room' Ventilation System Following Testing."
As indicated in the attached Licensee Event Report, HL&P is now in full compliance.
Corrective actions have been taken to prevent recurrence.
However, as noted in Corrective Action #3, a schedule is being developed to cover preparation of a formal training program for procedure writers and personnel responsible for procedure performance.
This schedule will be developed by July 31, 1989.

On January 7, 1989, Unit 2 was in Mode 6 prior to initial criticality..During
]
the performance of a surveillance test to verify operability of the Control
'
Room Envelope HVAC system, it was discovered that Train C did not function properly.
Further investigation revealed that plastic sheeting was installed over the makeup filter inlet during a test conducted on December 29, 1988 and had not been removed. This was a violation of Technical Specification 3.7.7.
The cause of this event was an inadequate test procedure which did not include J
specific steps for installation, removal, and independent verification of the
'
plastic sheeting.
This is in violation of existing plant administrative procedures.
To prevent recurrence, plant personnel responsible for preparation of procedures will be trained on the requirements to provide positive procedural control and independent verification of actions which alter the configuration of plant systems.

On January 7, 1989, Unit 2 was in Mode 6 prior to initial criticality.
During the performance of a surveillance test to verify operability of the Control Room Envelope NVAC system, it was discovered that Train C did not function At 0115 hours0.00133 days <br />0.0319 hours <br />1.901455e-4 weeks <br />4.37575e-5 months <br /> Operations personnel investigating the problem found properly.
plastic sheeting taped over the inlet of the supply duct to the Train C makeup carbon filter housing. Technical Specification 3.7.7 was entered pending removal of the plastic sheeting. At 0145 hours0.00168 days <br />0.0403 hours <br />2.397487e-4 weeks <br />5.51725e-5 months <br /> the sheeting was removed and the system was successfully tested and returned to service.
An investigation into the source of the plastic sheeting was conducted to determine the date of installation. It was discovered that the sheeting was installed to protect the control room carbon filters during the performance of

a Halon concentration test on December 29, 1988.
Since this resulted in the inoperability of the Train C Control Room HVAC System for more than the seven days allowed by Technical Specification 3.7.7, this event was determined to be 1638 hours0.019 days <br />0.455 hours <br />0.00271 weeks <br />6.23259e-4 months <br /> on reportable and the NRC was notified pursuant to 10CFR50.72 at January 10, 1989.
.The Halon concentration t st procedure was a temporary procedure intended for
'
one time use only.
It will be deleted from the plant procedure manual by February 22, 1989.
CAUSE OF OCCURREJCE:
include The cause of this event was an inadequate test procedure which did not detailed inst. ructions for the installation and removal of the plastic sheeting and did not include requirements for independent verification of these actions. This condition is a violation of existing plant administrative procedures which control preparation of test procedures.
ANALYSIS OF EVENT:
i Technical Specification 3.7.7 requires that an inoperable Control Room Makeup and Cle'anup Filtration System Train be restored to operable status within 7 days.
This condition resulted in Train C being inoperable for 9 days.
l Technical Specification violations are reportable pursuant to 10CFR50.73(a)(2)(1)(B).
At the time of this event, Unit 2 was in Mode 6 prior to initial criticality.
As such, no radioactivity had been produced and no decay heat was present.
Based on the surveillance interval defined in Technical Specification 3.7.7, the maximum time before the isolated train would have been detected was 31 days.
Therefore, in the worst case, the system would have been returned to service prior to initial criticality. The two redundant systems were operable throughout this event.

CORRECTIVE ACTION:
The following corrective actions are being taken as a result of this event:
1.
A bulletin will be issued to plant personnel responsible for preparation of procedures which will discuss this event and reinforce the need to provide positive procedural controls and independent verification of actions which alter the configuration of'
plant systems. This action will be completed by February 15, 1989.
2.
Interim training of plant personnel responsible for the preparation.

3.
A review of the need for a formal training program for procedure writers and personnel responsible for procedure performance will be completed by April 1, 1989. If additional training is determined to be necessary, a schedule for completion of the training will be developed at that time.

LER 87-012 Inoperable Feedwater Flow Transmitters Due to a Procedural Deficiency An extension of the 10CFR50.73 required due date was granted for this LER per discussion on February 6, 1989 with Mr. G. Holler of Region IV as a result of the effects of the inclement weather upon site nonessential staffing.
