l Inspection Sumary Inspection Conducted May 1 through 31, 1983 (50-482/83-14)
Areas Inspected: Routine, announced inspection including site tours; test program requirements; review of station procedures; and followup on unresolved items. The inspection involved 75 inspector-hours onsite by the Resident Reactor Inspector - Operations.
Results: Within the 4 areas inspected, one violation was identified

- The above identified personnel attended the exit' meeting held on May 19, 1983.

Facility Status On May 16, 1983, Kansas Gas and Electric Company announced several

changes in the station organization and also announced an accelerated fuel load date. At a meeting held by corporate management with all station supervisors (approximately 800 people), it was announced that Mr. Frank Duddy would assume the newly created position of Project Director.
In this position, everyone on the site, except quality assurance, reports to Mr. Duddy. He, in turn, reports to Mr. G. L.
Koester, Vice President-Nuclear, KG&E.
Also at this meeting, a new fuel load target date of August 15, 1984, was announced. This date is approximately eleven weeks earlier than previously schedule m

Site Tours The RRI toured the plant several times.during the month. The primary areas inspected assured compliance with: '(1) ' clearance order procedure; (2) test program requirements; (3) housekeeping; (4). logbooks; and (5) foreign material exclusion.
.
One violation was. identified in the area'of ' test program requirements.
This item is addressed in paragraph.4;of.this report.
Additionally,:the RRI cbserved the te'st run of the "A" reactor building spray pump.. This pump was run to. determine the cause of oil foaming in the motor bearings. A similar, problem was encountered at the Callaway site.
Final disposition has not yet been made.
. The RRI found that quality control " accept tags" are being removed from their designated components and are being placed on other equipment by persons unknown. This has been discussed with the licensee and some control fonn is to be developed.

Test Program Requirements During this period, KG&E startup personnel realigned the (NK) 125 volt DC Class 1E electrical system. This realignment was being done to support vendor rework of the battery chargers that have been experiencing i
numerous failures. The system changes were being made without benefit l
of procedure or, of any other startup controlled document.

The startup engineer was directing the activity from his memory of the system. The only control mechanism in use was the temporary modification procedure being used to identify the jumpers installed from the one battery charger to the other three systems.
KG&E startup personnel felt the temporary modification procedure was
'
enough to control this plant shange even through no review nor concur-
rence is required for temporar, inodifications of this sort. Although l
supervisory personnel knew of the change in plant lineup, the new
!
configuration had not been reviewed nor approved.
Furthermore, the l
control room operator that the RRI talked with knew that the startup
!-
engineer was doi.ng "something" to the NK system but he did not appear l
to know exactly what changes were being made.
This failure to provide procedures is an. apparent violation.

5.
Review of Station Procedures The RRI reviewed several administrative procedures. The purpose of this review was to ensure compliance with commitments. No significant concerns were raised by this review.
Procedures reviewed were:

No violations or deviations were-identified.
6.
Followup on Inspector Identified Unresolved Item (Closed) Unresolved Item (ST" 50-482/8222-06): Steam Generators -
this item remained unresolved awaiting responses from Aptech Engineering and Westinghouse concerning conditions inside the steam generators and pressurizer. The evaluations conducted by these organizations show no concern for the internal condition of these components. Westinghouse
_
stated the debris in the steam generators (secondary side) would be removed by normal demineralized water flushing prior to cold hydro.
I Due to the response being complete and accepted by the licensee, this item is considered closed.
7.
Exit Meeting l
The RRI held an exit meeting on May 19, 1983, to discuss the scope and findings of this inspection.

quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be t

drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that t
important activities have been satisfactorily accomplished."

The Wolf Creek Operation Quality Assurance Manual Section 17.2.5.4, Operations Documents requires:
"The Wolf Creek Generating Station

Contrary to the above, on May 18, 1983, KG&E startup realigned the (NK)
it 125 volt DC Class IE system.
The RRI observed all four NK busses were
being fed from one battery charger.
No procedure was available for this operation.
The systems were being aligned to support repairs to it the other battery chargers, all alignment was being done by the startup
