Licensee Actions on Previous Inspection Findings-(Closed) Violation (STN 50-482/8204):
Failure to perform timely cor-rective action relative to the modification and revision of Procedure No. AP-IX-03, " Document Control".
Procedure No. AP-IX-03, Revision 15, dated, June 2,1982, required that superseded documents be returned to the document control section within
'four working days. To prevent a recurrence of this violation, Pro-cedure No. AP-II-12, " Project Audit Response", was revised on April 15, 1982, to incorporate the use of an " Audit Finding Status Log" to specifically track and monitor the timeliness of audit responses.
(Closed) Violation (STN 50-482/8216):
Failure to follow procedures relative to the protection of stainless steel to prevent carbon steel i
contaminatio.

Piping installation and fabrication Procedure No. WP-VII-201, was revised to clarify that direct contact of stainless to carbon steel is not permissible during handling operations. To prevent a recur-rence of this violation, training was held for involved personnel on the requirements for handling of stainless steel during installation.

Other Findings and Comments On March 23, 1983, a DIC employee approached the NRC resident inspector and expressed concerns relative to:
(1) welding of electrical raceway supports, and (2) the inspection of expansion anchors. The NRC inspector performed a followup inspection and the results are as follows:
a.
Welding of Electrical Raceway Supports An inspection was performed on the installation of electrical raceway supports in the reactor building by the NRC inspector and the KG&E surveillance section.
Drawing No. C0402Q, Note 8, requires that welding and inspection be performed to the requirements of AWS D1.1-75.
An inspection of 10 supports, selected at-random, indicated that the intermittent. fillet weld configuration was not in accordance with the drawing details or the requirements of '
AWS D1.1, paragraph 4.6.1, in that the welds did not start or termi-nate at the ends of the dimensioned length.
It was also noted that four of the ten supports inspected had been' accepted-by DIC QC. DIC issued a Nonconformance Report (NCR) No. ISN 9130E on March 1,1983, which identified this discrepancy. NCR No. ISN 9130E was fomarded to Bechtel Engineering for disposition. However, neither the NCR nor the attached sketch, which was identified as a representative example, are indicative of the worst case conditions that exist. The NRC inspector observed certain support welds that were approximately 2 inches from the ends of the dimensioned' length.
The NRC inspector found that Procedure No. QCP-X-302, Revision 12,
" Installation of Electrical Raceway Supports", did not address the intermittent fillet weld requirements of AWS D1.1-75.
This is considered to be a violation of 10 CFR Part 50, Appendix B, Criterion V.

b.
Inspection of Expansion Anchors On February 17, 1983, a DIC inspector sent a memo to DIC quality engineering (QE) requesting clarification on the inspection criteria relative to the verification of the length of expansion anchors installed prior to August 12, 1982.
DIC QE responded on Febru-ary 21,1983, stating that anchor bolt length is not a required

inspection attribute for bolts installed prior to August 12, 1982, based on Bechtel Engineering disposition of NCR ISN 3476C. As a result of the review of the disposition, it would appear that it relieved DIC of inspection for abandoned expansion anchor holes only and not the other inspection attributes such as length, diameter, embedment, and thread engagement. A memo to all electrical quality inspectors dated March 25, 1983, rescinded the DIC QE response of February 21, 1983, and directed them to inspect expansion anchors for all attributes in accordance with Procedure No. QCP-X-302.
The HRC inspector expressed concern that DIC Quality Engineering made such a liberal interpretation of Bechtel Engineering disposition of NCR No. ISN 3476C.
No violations or deviations were identified.
4.
Safety-Related Piping The NRC inspector observed the fitup of piping for the chemical and volume control system to ascertain that welding was being controlled with respect to the following:
a.
Welding was being performed in accordance_with approved procedures which meet the. requirements of Section IX of the ASME Code including a review of the supporting procedure qualification record.

Welders were qualified for ~the process.being used relative to materials, position, and procedure.

Filler metals used were controlled and-traceable from the point of issue to the point of consumption and were i'entified to the required d

5.
Plant Tours Plant areas were _ toured several times during the reporting period to observe general construction practices, area cleanliness, and storage conditions of plant equipment.

6.
Exit Meeting The~ SRI met with licensee representatives denoted in paragraph 1, on April 7, 1983. The scope of the inspection activity and the findings were discussed with the licensee representatives. As a result of the discrepant welding found in the electrical raceway supports and the HVAC hangers, the SRI expressed concern relative to the quality of all AWS structural welding at the Wolf Creek Site.
