APPENDIX A NOTICE OF VIOLATION Duke Power Company License Nos. CPPR-116 Catawba Nuclear Power Station CPPR-117 Based on the NRC inspection April 3-6, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.

A.

As required by 10 CFR 50, Appendix "B", Criterion V and as implemented by Duke Power Company (DPC) Topical Report " Duke 1-A", Section 17, para-graph 17.1.5, activities affecting quality shall be accomplished in accordance with instructions, procedures, or drawings. DPC QA Procedure M-2, Rev. 15, " Inspection of Design Concrete", paragraph 4.6 requires in part that " Honeycombed, cracked and other defective areas needing repair shall be noted in the remarks block of Section 1 Form M-2N."

Contrary to the above, a void area approximately 30" long x 2" wide and 8" deep (maximum) had not been identified nor repaired after completion of the QC inspection of pour number 32 on the Unit I containment dome.

This is an infraction.

B.

As required by 10 CFR 50, Appendix "B", Criterion V and as implemented by Duke Power Company (DPC) Topical Report " Duke 1-A",

Section 17, para-graph 17.1.5, activities affecting quality shall be accomplished in accordance with instructions, procedures, or drawings. DPC QA Procedure L-72, Rev. 2, requires " good construction site housekeeping practices" for housekeeping zone VI.

The Unit 2 refueling canal was designated as a housekeeping zone VI.

Contrary to the above, on April 3,1979 a wooden platform in the Unit 2 refueling canal was covered with litter and debris, examples are:

ciga-rette wrappers, pop cans, damaged face shields, a hard hat liner, two weld rods, etc.

This is a deficiency.
