Construction Progress The inspector conducted a general inspection of the power block

construction site and the pipe fabrication shop, to observe construction progress and construction activities such as welding, nondestrictive examination, material handling and control, housekeeping and storage.

Inspector Followup Items a.

(Closed) Item 400, 401, 402, 403/81-03-02:

" Inspection records of equipment installation".

This item concerned the lack of equipment installation checkoff sheets to provide objective evidence that significant milestones were witr.essed and found to be in compliance with applicable requirements.

The licensee has:

revised TP-24,

" Mechanical Pipe Installation Inspection", and TP-28, " Inspection of Equipment Installation",

and issued TP-45, " Inspection of the Installation and Repair of Valves and Speciality Items", to address this concern.

The inspector has no further questions in this area, this item is considered closed.

" Visual indication in main steam /feedwater restraint frame" The licensee documented this condition in DDR-621, and reported it as CDR 81-060.

The inspector examined the frame and applicable documentation to assure that above matter was properly documented, reviewed and closed out.

(Closed) Item 400/82-01-02: " Welder instruction needed for reading gas flow meters" The licensee has instructed welders in the proper method of reading gas flow meters". The inspector has no further questions in this area, this item is considered closed.

(Closed) Item 400, 401/82-11-02: " Procedure ES-169 revised for control of temporary attachments" This item concerns the need for revision to RECO Procedure ES-169 to provide adequate control of temporary attach-ments. Procedure ES-169 has been revised to address this concern. The inspector has no further questions in this area, this item is considered closed.

(Closed) Item 400 CDR 80-043:

" Seismic Class 1 Hangers Insuf ficient Preheat".

CP&L has identified that insufficient preheat has been applied to the emergency service water pipe hangers located in the pipe run through the turbine building to the diesel building.

CP&L is investigating extent of problem and the effect on hangers.

Hangers were site fabricated. Af ter the initial notification and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable. This inspector reviewed / inspected the applicable deficiency reports, evaluation and the item in question.

Based on the above review and inspection this inspector concurs that the item is not report &ble.

(Closed) Item 400 CDR 80-045:

" Safety-Related Pipe Hanger Made With Fillet Weld in Lieu of Full Penetration Weld". The subject hanger was fabricated on site and the deficiency noted prior to the QC acceptance inspection. After the initial notification and prior to this inspec-tion the licensee stated that this condition was not considered significant and thus not reportable. This inspector reviewed / inspected the applicable deficiency reports, evaluation and the item in question.

Based on the above review and inspection this inspector concurs that the item is not reportable.

(Closed) Item 400 CDR 80-046:

" Flexing of Containment Dome During Move" In order to move the containment dome, cranes were located at the front and back opposite each other. However, the lead crane moved forward while the follow crane remained stationary causing a flexing of the head. Af ter the initial notification and prior to this inspection, the licensee stated that this condition was not considered significant and thus not reportable.

This inspector reviewed / inspected the applicable deficiency reports and evaluation of the items in question.

Based on the above review and inspection this inspector concurs that the item is not reportable.

d.

(Closed) Item 400 CDR 80-047:

" Anchor Plate Welds".

At receipt inspection, cracks were noted in vendor welds joining carbon steel concrete anchor supports to stainless steel embeds. These embeds are for Unit I and are supplied by Alfab. After the initial notification and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable. This inspector reviewed / inspected the applicable deficiency reports, evaluation and a representative sample of the items in question.

Based on the above review and inspection this inspector concurs that the item is not reportable.

(Closed) Item 400 CDR 80-054:

"Cadweld Sleeves Welded Without Qualified WPS" At two locations, cadweld sleeves were welded together using a welding procedure specification that was not qualified for the cadweld sleeve material involved (i.e., SA-519). One location was at containment feedwater penetration number four (261' elevation - 278 azmuth) and the other was in the containment exterior wall (252'

elevation - 90 azmuth). After the initial notification and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable.

This inspector reviewed / inspected the applicable deficiency reports, evaluation and a representative sample of the items in questions. Based on the above review and inspection this inspector concurs that the item is not reportable.

(Closed) Item 400 CDR 81-057;

" Metal Clip for Walk. ways Inside Containment" At elevation 285, azmuth 328, a clip welded to an imbedded steel plate has a linear indication of fault caused by inclusions in the steel.

This was found while preheating the clip.

The clip has full length (IS") linear indication. The steel type is A36. Af ter the initial notification and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable. This inspector reviewed /insoected the applicable deficiency reports, evaluation and a representative sample of the items in question. Based on the above review and inspection this inspector concurs that the item is not reportable.

(Closed) Item 400 CDR 81-060:

" Linear Indications in Main Steam and Feedwater Pipe Frames" Licensee discovered linear indications in material ( A36 metal) used for pipe hanger frames used for main steam and feedwater pipe. Frames are manufactured by Southwest Fabricating and are seismic qualified. Af ter the initial notificatica and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable.

This inspector reviewed / inspected the applicable deficiency reports, evaluation and a representative sample of the items in question.

Based on the above review and inspection this inspector concurs that the item is not reportable.

(Closed) Item 400 CDR 81-064: " Excessive Weld Gap on Steam Generator Piping Structure" The loop 3 steam generator piping restraint structure has excessive gaps in the butt weld area.

Seven butt welds performed by Pittsburgh Bridge and Iron have gaps ranging from 3/16-inch to 1/2-inch and the allowable is 1/8-inch gap.

After the initial notification and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable.

This inspector reviewed / inspected the applicable defi-ciency reports, evaluation and a representative sample of the items in question.

Based on the above review and inspection, this inspector concurs that the item is not reportable, i.

(Closed) Item 400 CDR 82-069: " Seismic I Pipe Hangers With Undersize Welds" Welds on some of the 187 previously accepted Seismic I pipe hangers were subsequently found to be rejectable; the primary problem being that the welds are undersized.

The QA/QC inspector, who originally performed the inspections on those 187 Seismic I pipe hangers and accepted the welds, has since been terminated. Af ter the

(0 pen) Item 400 CDR 82-071:

" Embedded Strip Plates Received With Inadequate Welds on Studs".

On February 16, 1982, the licensee notified IE:II of a 50.55(e) item concerning defective embed plate stud welds. The final report was submitted on April 15, 1982. The report has been reviewed and determined to be acceptable by IE:II.

The inspector held discussions with responsible licensee and/or contractor representatives, reviewed supporting documentation, and observed representative samples of work to verify that the corrective actions identified in the report have been completed. The final report does not address the issue of stud weld quality on Alfab embea plates received prior to January 18, 1982. The licensee indicated that they would amend the final report to address this issue. This item remains open.

(Closed) Item 400 CDR 82-073: "Substandarc 1/4-Inch Red Head Concrete Anchors".

The licensee has determined that substandard non-nuclear grade 1/4-inch Red Head concrete anchors have been supplied by the vendor, ITT Phillips. Approximately 20. of the boxed warehouse stock of these anchors are made of AIFI 1213 material rather than the AIFI 1144 material as specified. The licensee is investigating to determine the cause and extent of the problem. There are approximately 172 such anchors presently installed at Harris.

After the initial notification and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable. This inspector reviewed / inspected the applicable deficiency reports, evaluation and the item in question. Based on the above review and inspection this inspector concurs that the item is not reportable.

(Closed) Item 400 CDR 82-074:

" Unsatisfactory Welds on RHR Heat Exchanger" During weld inspection on RHR heat exchanger 2168-183, it was determined that an overlap weld on the RHR side may be unsatis-factory. The unit was supplied by Joseph Oats of Camden, New Jersey.

After the initial notification and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable. This inspector reviewed / inspected the applicable deficiency reports, evaluation and the item in question. Based on the above review and inspection this inspector concurs that the item is not reportable.

(Closed) Item 401 CDR 82-078: " Improper Storage of Main Steam Lines".

Licensee reported that sections of three main steam lines in storage at the site were observed to be improperly supported. After the initial notification and prior to this inspection the licensee stated that this condition was not considered significant and thus not reportable. This inspector reviewed / inspected the applicable deficiency reports, evalu-ation and a representative sample of the items in question. Based on the above review and inspection this inspector concurs that the item is not reportable.

Reactor Vesse! Installation (Unit 1)

The inspectors reviewed procedures and quality records to ascertain whether procedures, field activities and records pertaining to the installaticn of the reactor vessel were consistent with NRC requirements, applicable codes standards and commitments.

The reactor vessel was to be fabricated from steel plate conforming to ASME SA-533 G B.CL.1.

Review of Quality Records The inspector reviewed the Certified Material Test Reports (CMTR) for the plate materials listed below to ascertain whether the records are in conformance with applicable codes, standards, and commitments.

With regard to the inspection above, the inspector noted that the analysis percentages on the CMTRs for heat numbers C-9924-1 and C-9924-2 and the dropweight test temperatures on the CMTR for heat number A-9553-1 were enhanced with a pen for legability. The ent:ance-ment has obliterated any trace of the original print, therefore there is only limited assurance of the actual values. The licensee indicated that they would look further into the matter.

With regard to the above inspection the inspector noted that there was no copper analysis for heat numbers C0224-1 and C0123-1.

Paragraph 5.3.1.1 of the FSAR requires copper analysis for reactor vessel beltline material.

At the time of this inspection it could not be determined whether the above material is included in the beltline. The licensee indicated they would look further into the matter.

The inspector stated that the above would be identified as unresolved item 400/82-18-02, " Copper Analysis" Within the areas examined, no violations or deviations were identified.
