Facility representatives then responded to NRC questions concerning the root cause analysis.
Facility representatives also stated that they w'ere not able to present the details of their planned corrective actions, since those were still under development.
They stated that they had not yet looked beyond those steps which were considered necessary to permit the operators who had been removed from licensed duties to be returned to licensed duties.
It was agreed that these details would be discussed at a subsequent meeting.

NRC representatives questioned the licensee's assertion that ineffective licensee coordination with the NRC had been a
cause for failure of the requalification program evaluation.
The NRC pointed out that there had been an extensive history of meetings, which the licensee had attended, to notify the licensee of changes in the requalification process.
Moreover, it appeared to the NRC that the failures which had occurred would have occurred under the previous revision of the process as well, since they related to fundamental EOP performance error Wt

Licensee representatives stated that

NRC ri. ~resentatives questioned the commitment of operators to folLow emergency operating procedures (EOPs),
and licensee management's commitment to ensuring that EOPs were followed.
In addition, the NRC representatives questioned whether licensee management had provided sufficient oversight to the requalification training program in the last year.
These points were discussed in detail.
Licensee management acknowledged that the requalification examination was not any more difficult than previous examinations.
Management maintained, however, that unexpected changes in the examination process had resulted in rumors or statements by unknown persons which had significantly increased the operator's stress level to the extent that they had performed more poorly than their capability.
The NRC representatives expressed concern regarding unnecessary stress being placed on operators and asked the facility what actions it had taken to stop the flow of rumors from training to operations.
The NRC also asked the licensee how stress would have lowered operator's threshold for departing from their EOPs, since it was not.
clear how these two ideas were connected.
It was apparent from some of the responses to the NRC questions that the Supply System and the NRC did not share a
mutual'nderstanding of the root causes of the Supply System's unsatisfactory requalification program.
Mr. Martin summarized the NRC's concerns.
He stated that in order to be satisfied that the plant was safe,to restart from the current refueling outage, it was necessary for the license'e to ensure that licensed operators could, and would, use their EOPs, and to ensure that an effective training program to maintain operator competency was in place.
He emphasized that an additional meeting to further discuss these concerns was necessary.
Mr. Mazur stated that the Supply System clearly understood this message, agreed that another management meeting to discuss the points made by the NRC would be appropriate, and asserted that the Supply System would fix its problems.
Mr. Russell outlined the recent procedure for a requalification program to be identified as provisionally satisfactory once the NRC's confidence has been restored that a facility can effectively train and evaluate its licensed operators.
