SUMMARY OF MEETING BETWEEN NRC AND WPPSS ON MAY 20, 1991 AT WALNUT CREEK, CALIFORNIA (REPORT NO. 50-397/91-19)
This letter is to summarize the meeting held in the NRC Reqion V office on May 20, 1991 between senior NRC management and Washington Publ)c Power Supply System management (Enclosure 1).
The purpose of the meeting was to discuss the results of your reassessment of the root causes of your requalification program and your corrective action plans, presented by your letter to us dated Hay 16, 1991.
From our discussions and your presentation it appears that you have objectively assessed the root causes and determined appropriate corrective action plans.
We urge you to continue your aggressive implementation of those plans and, particularly, to periodically assess the effectiveness of your corrective actions.
We will monitor your progress and assess the effectiveness of your corrective actions in upgrading your training programs.

The NRC and Supply System held a management meeting on May 2, 1991, at the Region Y office.
The purpose of that meeting was to discuss the root causes of the requalification program failure and the Supply System's corrective action program for the deficiencies identified.
The minutes of that meeting were documented in NRC inspection report no. 50-397/91-14.
It was apparent during the May 2, 1991 meeting that the Supply System and the NRC did not share a mutual understanding of the root causes of the requalification program failure.
Hence, the Supply System and NRC agreed to hold another management meeting to discuss the root causes and corrective actions after the Supply System reassessed the circumstances surrounding the requalification program failure.
The licensee performed a reassessment and provided the results to the NRC by letter no.
G02-91-102, dated May 16, 1991, titled
"Licensed Operator Requalification Program Corrective Action Plan."

The purpose of this Hay 20, 1991 meeting was to discuss the licensee's reassessment of the root causes of the requalification program failure and the corrective action plans provided by the Hay 16, 1991 letter.
The Supply System opened the meeting with the results of their assessment of the causative factors.
These were:
a casual attitude of operators toward compliance with emergency operating procedures (EOPs),
training was not adequately driving EOP performance and maintaining operator skills, and management did not recognize and take appropriate, actions to address these concerns.
The Supply System then presented their corrective action plans, as detailed by their Hay 16, 1991 letter.'he presentation was conducted using the attached visual aid outline.
Items discussed included the Supply System's short, intermediate and long term actions, as detailed in their letter.
Dr. Bouchey briefly discussed the plans for continued equality Assurance oversight of licensed operator training programs.
The scope of the presentation is also provided by tlie attached visual aid outline.
The Supply System also presented a proposed schedule for achieving a
provisionally satisfactory requalification program and conducting another NRC administered requalification examination.
Hr. Martin observed that the Supply System's reassessment of root causes and corrective action plans appeared to be more focused on the problems that led to the unsatisfactory performance by the MNP-2 licensed operators.
Regarding the licensee's proposed schedule, Mr.
Hartin noted that a necessary ingredient is the performance of the operators on the requalification retake examination, currently scheduled for early June, 1991.
Mr. Martin closed the meeting by stating that the NRC will review the licensee's corrective action plans, discuss these internally, and get back to the Supply System if the NRC has any question l
