Inspection on May 29-31 and June 1, 1979 (Report No. 50-382/79-06)

Areas Inspected:

Routine, unannounced inspection of safety related construction activities related to a follow-on review of a previously identified item of noncompliance; a status review of a previously reported construction deficiency; the review of electrical procederes and QA/QC records; observation of electrical installation work activities; and the review of an unresolved item.

The in-spection involved twenty-five inspector-hours by one NRC inspector.

Results: No items of noncompliance or deviations were identified.

r Mounting / Installation of Class IE,160 Volt Switchgear. The IE inspector reviewed Fischbach & Moore Inspection Report No. 316-28-59 and Nonconformance Report (NCR) No. W3-1294.

Ebasco is now reviewing the "as-built" installation of the switchgear to determine seismic stability.

The IE inspector also discussed Fischbach & Moore organi-zational changes which were discussed in LP&L's response letter of April 27, 1979.

This item will remain open pending final resolution of NCR W3-1294.

(Open) Construction Deficiency Report:

Electrical Penetration Support Frame, September 27, 1978. The IE inspector reviewed NCRs No. W3-1057 and W3-1261, Design Change Notice E-229, and Field Change Request (FCR)

E-911.

Fischbach & Moore will perform the additional welding to secure the pemnut fasteners. This item will remain open pending implementation of FCR E-911.

(Closed) Unresolved Item (50-382/79-02, Paragraph 3):

Seismic Qualification of Rigid and Nonrigid Contact of Electrical Raceway and Electrical Equipment.

Based on a meeting between Dr. Z. T. Shi (Ebasco) and the IE inspector on March 7, 1979, and a May 9, 1979, Ebasco memo from D. Sandiforth to W. C. Griggs documenting the conclusions of the meeting on March 7, 1979, this item is considered closed.

The rigid response curves of a given floor for a frequency of 6 Hz to 33 Hz are flat and the

"g" values are practically equal to a zero period of acceleration. The conc ~lusion is that the loading of the rigid and nonrigid connections are insignificant and need not be considered in the seismic qualification of Class IE cabinets.

Plant Tour The IE inspector walked through the Reactor Containment, Reactor Auxiliary Building, warehouses, and various storage areas to observe construction activities in progress and to inspect the general state of cleanliness and adherence to housekeeping requirements.

The IE inspector also discussed a small fire that took place inside the containment. On May 25, 1979, a preheat controller box shorted out starting a fire in some temporary cabling. The fire was extinguiched by site per-sonnel.

The IE inspector examined photographs of the fire affected area and inspected the fire affected area during the plant tour.

Combustion Engineering is evaluating possible damage to some mirror insulation in the area.

No items of noncompliance or deviations were identified.

Electrical Systems and Components The IE inspector reviewed the following procedures, drawings, and records for cable raceway and cable installation using 10 CFR 50, Appendix B, FSAR Section 8, IEEE 384, and Regulatory Guide 1.75 as the acceptance criteria.

F&M RFI-1540 Ebasco DCN-NY-E-231-1 Field Change Requests E-819D, E-889 and E-1032 F&M Inspector Training Records The IE inspector discussed the findings of LP&L QA Audit Report W35 79-28 of May 3, 1979, with the licensee representative.

The licensee represen-tative advised the IE inspector, that as a result of the aforementioned audit, Ebasco is now doing an extensive review and redesign of raceway and cable installation in the cable spreading area.

The IE inspector also reviewed the following procedures, instructions, and records relating to maintenance of Class IE equipment being performed by Gulf Engineering:

CMI 54/2, " Care and Maintenance Instruction for Containment Spray Pumps and Motors" CMI 59/3, " Care and Maintenance Instruction for Component Cooling Water Pumps and Motors" CP 425, " Insulation Resistance Testing of Rotating Electrical Equipment" PR. 9.2,'~ev.

PR. 9.C, Rev. 1, " Handling, Storage and Preparation Equipment" Maintenance and Inspection Records for Cont. Spray Pumps A, B and AB; and Component Cooling Water Pumps A, B and AB The above procedures and records appeared to comply with the requirements of 10 CFR 50, Appendix B, accepted engineering practices, and ANSI N45.2.

The IE inspector's review of the Design Change Notice DCN-NY-E-231-1 indi-cated that Ebasco appeared to be deviating from the separation criteria specified in IEEE 384, 1974, and Regulatory Guide 1.75, 1975.

DCN-NY-E-231-1 states that where the one inch separation between fire barriers for redundant raceways crossing each other cannot be maintained, a one-eighth inch blanket of Quelpyre Mastic Type 703B can be applied to the lower raceway in lieu of providing the one lech free air space. The IE inspector asked the licensee representative if an analysis, in accordance with IEEE 384, paragraph 5.1.1.2,

had been accomplished for the use of one-eighth inch Quelpyre Mastic in lieu of one inch free air spac, The IE inspector was informed that this infor-mation would be forthcoming from Ebasco, New York. As of the close of this inspection, this information was not available.

This is an unresolved item pending review of the analysis required by IEEE 384 by the IE inspector.

5.

Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance, or deviations.

One item relating to DCN-NY-E-231-1, Separation Criteria for Redundant Raceway, is identified in paragraph 4.

6.

Exit Interview The IE inspector met with the licensee representatives (denoted in paragraph 1) at the conclusion of the inspection on June 1, 1979. The IE inspector summarized the scope and findings of the inspection.
