Areas Inspected:
Routine, unannounced inspection of construction activities related to a follow-on review of QA/QC documentation recorded during the final setting and installation of the reactor vessel; a review of licensee correspondence relative to ASME, Class 3, Code commitments; and a review of
' corrective action and schedule impact relative to HVAC seismic support hanger weld problem. The inspection involved twenty-eight inspector-hours by one NRC inspector.

Licensee Action on Previous Inspection Findings (0 pen) Construction Deficiency Report:
Lack of pre-heat during welding HVAC seismic support hangers.
The licensee submitted an additional interim report, dated December 29, 1978, which contains the results of an evaluation study performed by Ebasco and indicating that 425 seismic supports, welded with E-6011 electrodes without pre-heat, are unacceptable.
During this inspection the IE inspector discussed the matter with the cognizant licensee representatives to determine the extent of repair required and the potential impact on the overall construction progress.
The licensee representatives stated that the repairs should not impact on the overall schedule and that the HVAC contractor has been requested to provide a detail schedule by January 31, 1979, reflecting the completion of repairs by October 15, 1979.

Reactor Vessel - Final Setting and Installation - QA/QC Documentation The IE inspector conducted a follow-up review of the final setting of the reactor vessel QA/QC documentation records. Documents reviewed-2-

included the Nuclear Installation Services Company (NISCO) procedure ES-159, " Reactor Vessel Final Setting Procedure," revision A, dated December 5, 1978; eight (8) Process Control Sheets - PCS3015-150; Reactor Vessel Support Reference Drawings; and Eleven (11) as-built data sheets attached to the process control sheets.
In reviewing the accumulated as-built data, recorded on the eleven data sheets attached to the process control sheets, the IE inspector observed that the process control sheets did not identify the attachments, nor were there any references established that would relate to the attached data.
In addition, verification requirements prescribed by NISCO procedure ES-159 were not clearly traceable to the process control sheets.
In discussing this matter with the NISCO representatives, the IE inspector indicated that the format of records documentation was not consistent with requirements prescribed by EBASCO procedure ASP-1-6,
" Quality Assurance Records" and ANSI N45.2.9 regiurements. Subsequent discussions held by the IE inspector with LP&L, EBASCO, and MISCO representatives revealed that the specific records were not in there final form and that the overall records documentation format established by NISCO would be reviewed and modified were necessary. The IE inspector stated that the matter would be identified as an unresolved matter and will be reviewed during a subsequent site inspection.

Review of Licensee Committments to ASME Section III, Class 3 Piping and Components During this inspection the IE inspector conducted an independent review of the licensee's committments to the intent of Section III of the ASME Boiler and Pressure Vessel Code (1971 Edition), safety class 3 components and piping.

Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance, or deviations. One item which relates to format of records documentation is identified in paragraph 3 above.

Exit Interview The IE inspector met with licensee representatives (denoted in para-graph 1) at the conclusion of the inspection on January 12, 1978 The IE inspector summarized the purpose and the scope of the inspection and the findings.
