Materials and Processes-Section Inspection Summary Inspection on February 25-26, 1981 (Report No. 50-374/81-06)

Areas Inspected: Review of welding and related procedures for installation of reactor coolant pressure boundary piping; observation of installation and welding of reactor coolant pressure boundary piping; review of material and personnel certifications; review of shop fabrication and field installation documentation; review of shop fabrication radiographs. The inspection in-volved a total of 17 onsite inspector-hours by one NRC inspector.

Observation of Activities The inspector observed 'the following weld joints being fit-up, fusing of the insert, and welding of one filler pass.

Weld Joint No. 2270, Recirculation System loop "B".

24" replacement pipe spool, S/N 6901-1P, heat No. 27557. Welded to 90* elbow identified as G.E. Pc.5, 2RR01AB. Welding Procedure P8-18LS, SMAW and P8-19LS, GTAW.

Weld Joint No. W-RH-4574-18", Component No. 1 2RH-14A, item 22 pipe welded to component No. 2, valve 2E12-F021. Welding Procedure F1-11LS, Revision 7 and P1-8LS, Revision 6 Insert, E70S-2, heat No. 659A315.

The following partially completed welds were visually examined during the inspection.

Weld No. 633M, Recirculation System loop "A", 90* spool to nozzle safe end at vessel 90* azimuth. Component No. 1, SNK-1141-00'e, pipe.

Component No. 2, N2 nozzle inlet.

Weld No. 629M, Recirculation System loop "A",

120* azimuth.

Component No. 1, pipe spool IEN-K-1140-P003 to component No. 2, G.E. Pc. No. 38, 2RR01CD.

Component No.~ 1, SN-K-1139-P002, (pipe). Component No. 2, 2RR01CE (pipe G.E. Pc.39).

Weld No. 635M, Recirculation System loop "A" 150 azimuth. _ Component No. 1, SN-K-1139-P002 (pipe). Component No. 2, N2 nozzle inlet.

During the examination of the above weld joints, the following was observed.

Documentation Review Documentation on Recirculation System replacement spools supplied by General Electric (GE) to Specification No. 22A6417 were reviewed.

GE Pc. Mark 112D2706P002 112D2706P003 112D2706P004 112D2706P005 112D2706P006 112D2706P007 112D2706P008 112D2706P009 112D2706P0010 112D2706P0011 All assemblies were manufactured from the same heat of material, heat No. 69133 rolled and welded plate.

Replacement spool, Pc. Mark No. 73/57435, 24" X 1.140" wall X 2'~1/4" long Heat No. 27657, SA240 Type 316 rolled and welded plate.

During documentation review, it was noted that the spools welded to each end of the 90* elbow making up the complete assembly were not identified with individual serial numbers, as many as four spools had the same number. Further, radiographs of the longitudinal welded seams could not be identified to the spools.

This is being identified as an unresolved matter pending further information from G.E.
