Inspection on September 1-4, 8-11, 14-15, 21-25, and 28-30, 1981 (Report No. 50-358/81-25)
l Areas Inspected: Routine safety inspection of previously identified items, preoperational test program implementation controls, quality confirmation program, fire prevention / protection, quality document examiners, and plant tours. This inspection invcived a total of 110 inspector-hours onsite by two NRC resident inspectors, including 10 inspector-hours onsite during offshifts.
Results: Of the six areas inspected, no items of noncompliance or devia-tions were noted.

Licensee Action on Previously Identified Items (Open) Unresolved Item 81-15-02: Single records control procedure 06-QA-02, revision 0, vill not support full scale construction activities.
The inspector reviewed revision 04 to the records control procedure 06-QA-02.
Also, several H. J. Kaiser QA-documentation instructions have been issued, dealing with the control of construction quality records.
(Open) Noncompliance 81-18-01: Failure to identify and correct pro-cedural nonconformances related to system turnover for preoperational testing rnd the system index test matrix.
The inspector verified that no systems had been turned over for pre-operational testing since the date of CG&E audit #357, that SU.PRP.01 was revised to require the QA Managers' signature on all preoperational tes, release packages prior to acceptance by the Nuclear Production

Department, that a procedure had been developed and implemented for QA review of preoperational test release packages, that the startup manual had been recently revised to more clearly define responsibili-ties and interfaces (as well as bringing the manual up to date), and that procedure revisions were in progress for SU.PRP.06 and Admin-2 to reflect the new startup manual requirements.

Preoperational Test Program Implementation Controls The inspectcr verified that jurisdictional controls (including
,
jurisdictional tagging, turnback to construction, and document freeze) were being observed for the nuclear boiler system; that the turnover coordination group was conducting an ongoing audit program to ensure that turned over systems remain tagged; that a schedule for preoperational testing was being maintained and kept current; that maintenance activities performed on systems turned over for preoperational testing were being performed in accordance with a technically adequate procedure by qualified personnel, and that preoperational test results had not been invalidated by maintenance activities.

Findings (1) A spot check of the tags in preoperational turnover PT-NB-01 revealed that all tags were in place and the tag hanging had been recently audited by the turnover coordination group.
The inspector noted that in the case of tags hung by I&C technicians, the tag listing had not been annotated with the date and persons initials as required by the procedure (SU.PRP.12).
(2) The inspector discussed QA review of turnover packages with the cognizant quality engineer and found that no turnover packages have passed the QA review to date. The review was being conducted in accordance with a procedure (06-QA-03)
and the individuals conducting the review appeared to be both knowledgeable and conscientious.
(3) The quarterly preoperational test schedule was in process of revision at the time of this inspectign and was being maintained under the direction of the test coordinator.

(4) The inspector observed maintenance performed on the motor controller for the auxiliary steam condensate pump 2A.
The maintenance was conducted in accordance with the approved procedure by qualified and experienced maintenance technicians.
No items of noncompliance or deviations were noted.
4.
Quality Confirmation Program - Review and Processing of Voided Nonconformance Reports The inspectors reviewed thirty-four (34) " Voided" Henry J. Kaiser (HJK) Nonconformance Reports (NRs). The review consisted of insur-ing that information on the original voided NR was exactly trans-lated onto a new " reopened" NR with a new control number. The final approval for all actions taken on a reopened NR shall be by a review committee, chaired by the CG&E QA Manager.
No items of noncompliance or deviations were noted.
5.
Fire Prevention / Protection The inspector verified that fire prevention system surveillances were being conducted in accordance with a schedule; that fire fighting equipment was in place in the control room, cable spreading room, on the refuelirsg floor, and in selected areas of the reactor building, and had been recently tested; that control room panel interiors and the cable spreading room were generally free of combustible materials; and that no obvious fire hazards existed in selected areas of the reactor building. The inspector also reviewed the results of inde-pendent fire inspections performed since the last inspection of this area to ensure that appropriate action was in progress to correct identified deficiencies.

Findings (1) The findings in QC FP-51 had received a timely reply from the responsible organization and corrective action was either planned or complete.
(2) Review of the ANI Annual Loss Prevention Report revealed that no new concerns were raised in safety-related areas.
(3) No significant fire hazards were identified. All fire pro-tection equipment observed by the inspector was in good condition and recently tested with the exception of one ABC dry chemical extinguisher. This extinguisher was identified by the constructors safety group and was replaced.
No items of noncompliance or deviations were noted.
6.
Quality Document Examiners The inspector reviewed Henry J. Kaiser Co. plans for qualification and certification of quality document examiners. The requirements are clearly established in procedure QADI #2, Revision 0, Certifica-tion of Document Examiners, dated September 25, 1981. This procedure reflects the requirements of ANSI N45.2.6-1978. The inspector dis-cussed the minimum qualification requirements with management ptrsonnel and was satisfied that qualified individuals would be used as quality document examiners.
No items of noncompliance or deviations were noted.
7.
Plant Tours The inspectors conducted frequent plant tours throughout this inspec-tion period. No new items of concern were identified during this period.
8.
Management Exit Interview The inspectors met with licensee representative (denoted in para-graph 1) at the conclusion of the inspection on October 2, 1981.
The inspectors summarized the scope and findings of the inspection.
