(Closed) Violation 338,339/85-28-01, Failure to Control Temperature of Concrete. Test Cylinders. During the referenced inspecition, the

inspector observed that -concrete test- cylinders in the field and laboratory curing room were not being controlled in accordance with the temperature requirements of ASTM C.3 Review of records during this inspection.showed that concrete design ,

,

. strengths were achieved even though temperatures were not being i controlled in accordance with ASTM C.31 requirements. Observations during this inspection showed that corrections to the laboratory curing

, room- water and cooling systems has resulted in proper temperature controls for laboratory cured test cylinders.- Observations also showed

,

that the licensee has placed curing boxes in the field which have heating and cooling controls and high-low thermometers for monitoring temperature controls. These corrections assure that curing of test

! cylinders will meet ASTM C.31 requirements. This item is closed.

l (Closed) Violation 338, 339/85-28-02, Failure to Control Moisture i

Content of Select Fill. Review of records during the NRC inspection conducted during October 28 - November 1,1985, indicated that the select fill moisture was not controlled to 3 percent of the optimum moisture requirements as specified by Specification NAS3003, Excavation Fill and Backfill Service Water Buried Piping Installation.

I As a result of this finding, test pits were excavated into the backfill and the material was retested. Test results showed that the inplace fill met the requirements of Specification NAS3003. Procedures were also revised to control the testing of the backfil Review of procedures and inspection records during this inspection showed that specification requirements are being adhered to and that problems with l

the backfill is being controlled in accordance with requirements. This I item is close (Closed) Violation 338, 339/85-28-03, Failure to Meet Specified Concrete Test Requirements for Slump, Air Content, Temperature and Aggregates and Document Correct Quantities of Concrete Mix Ingredients on Batch Tickets. Review of records during the October - November 1985 inspection showed that tests for slump, air content and temperature were not being made for each truck of concrete delivered as required by Specification NAS2014, Supply of Materials, Batching, Mixing, and Delivery of Ready Mix Concrete. The review also showed that the amount

This procedure was not appropriate in that it did not reference the current specifications being used to control the ongoing modification Review of procedures during this inspection showed that adequate procedures have been developed to control work and inspection activities for modifications to the service water intake structur This item is closed. Modifications to Service Water Intake Structure This inspection covered examination of specifications, procedures, and records, and observation of work activities for ongoing modifications to the Service Water Intake Structure in the areas of structural concrete and foundations. Records examined included inspection reports, concrete test data, backfill test results, nonconformance reports, concrete aggregate test data, certification records for cement, admixtures and rein forcing steel and calibration records for concrete and soils testing. equipment and the concrete batch plant. Examination of these records showed that inspections, testing, calibration of equipment, and receipt inspection of materials were being documented in accordance with specifications and procedures and that j problems were being identified and correcte Observation of work activities included ongoing concrete and grout place-ments, slump air and temperature tests on concrete mixes, making of concrete test cylinders, temperature controls for concrete test cylinders, vibration of concrete placements and batch plant operations. The inspector also observed inspection activities being conducted by construction engineers, and QC inspector Observations showed that construction work and inspection activities and documentation of work activities were being performed in accordance with specification and procedure requirements, that adequate specifications and procedures were in place for the control of these activities, and that management staff was actively involved in assuring that ongoing modifications were being conducted in accordance with requirements.
