On December 31, 2021, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Edwin I. Hatch. On January 21, 2022, the NRC inspectors discussed the results of this inspection with Sonny Dean and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Edwin I. Hatch, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. PLANT STATUS

Unit 1 began the inspection period at 100 percent rated thermal power (RTP). On October 10, 2021, the unit was down powered to 77 percent RTP to perform a rod pattern adjustment, the unit was returned to 100 percent RTP on the same day. On October 23, 2021, the unit was down powered to 96 percent RTP to perform a rod pattern adjustment, the unit was returned to 100 percent RTP on the same day. On November 6, 2021, the unit was down powered to 77 percent RTP to perform a rod pattern adjustment, the unit was returned to 100 percent RTP on the same day. On November 20, 2021, the unit was down powered to 87 percent RTP to perform a rod pattern adjustment, the unit was returned to 100 percent RTP on November 21, 2021. On December 4, 2021, the unit was down powered to 75 percent RTP to perform a rod pattern adjustment, the unit was returned to 100 percent RTP on December 5, 2021. On December 21, 2021, the unit reached maximum core flow and began the coast down in power for the 2021, spring refueling outage. On December 29, 2021, the unit was manually scrammed due to pressure oscillations caused by a failed pressure transmitter. The unit remained shut down for the remainder of the inspection period.

Unit 2 began the inspection period at 100 percent RTP. On November 30, 2021, the unit was down powered to 96 percent RTP to perform control rod drive, hydraulic control unit maintenance, the unit was returned to 100 percent RTP the same day. On December 10, 2021, the unit was down powered to 30 percent RTP to perform repairs to 2A adjustable speed drive (ASD), after repairs were completed the unit was returned to 100 percent RTP on December 15, 2021. The unit operated there for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, conducted routine reviews using IP 71152, Problem Identification and Resolution, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

71111.01 - Adverse Weather Protection

Seasonal Extreme Weather Sample (IP Section 03.01) (1 Sample)

(1) The inspectors evaluated readiness for seasonal extreme weather conditions prior to the onset of below freezing temperatures for the following systems on November 24, 2021:

The inspectors evaluated system configurations during partial walkdowns of the following systems/trains:

(1) Alignment of the Unit 1 residual heat removal (RHR) system 'B' and 'D' pumps during maintenance outage of loop 'A' RHR system, using 34SO-E11-010-1, "Residual Heat Removal System," Version 46.0, on October 7, 2021.
(2) Alignment of the Unit 1 reactor core isolation cooling (RCIC) system during maintenance outage of the high pressure coolant injection (HPCI) system, using 34SO-E51-001-1, "Reactor Core Isolation Cooling (RCIC) System," Version 30.4, on October 20, 2021.

Fire Area Walkdown and Inspection Sample (IP Section 03.01) (5 Samples)

The inspectors evaluated the implementation of the fire protection program by conducting a walkdown and performing a review to verify program compliance, equipment functionality, material condition, and operational readiness of the following fire areas:

(1) Unit 1 and Unit 2 refueling floor 228' elevation on October 6, 2021.
(2) Unit 1 and Unit 2 control building 180' elevation on October 8, 2021.
(3) Unit 1 reactor building 185' elevation on October 12, 2021.
(4) Unit 2 reactor building 130' elevation on October 15, 2021.
(5) Unit 1 reactor building 203' elevation on November 22, 2021.

71111.11A - Licensed Operator Requalification Program and Licensed Operator Performance

Requalification Examination Results (IP Section 03.03) (1 Sample)

(1) On November 23, 2021, the licensee completed the annual requalification operating tests and biennial written examinations required to be administered to all licensed operators in accordance with Title 10 of the Code of Federal Regulations 55.59(a)(2),

"Requalification Requirements," of the NRC's "Operator's Licenses." During the week of December 6, 2021, the inspector performed an in-office review of the overall pass/fail results of the individual operating examinations and the crew simulator operating examinations in accordance with Inspection Procedure (IP) 71111.11, "Licensed Operator Requalification Program." These results were compared to the thresholds established in Section 3.02, "Requalification Examination Results of IP

The inspectors reviewed and evaluated the licensed operator examination failure rates for the requalification annual operating tests and the biennial written examinations, which the licensee completed administering on November 23, 2021.

71111.11Q - Licensed Operator Requalification Program and Licensed Operator Performance

Licensed Operator Performance in the Actual Plant/Main Control Room (IP Section 03.01) (1 Sample)

(1) The inspectors observed and evaluated licensed operator performance in the Control Room during the pressurizing of the Unit 1 HPCI system steam piping, the HPCI maintenance run following electronic governor adjustment and valve maintenance, the operating of both loops of RHR and RHRSW in suppression pool cooling and emergency shutdown of HPCI turbine due to a leak from the main pump's mechanical seal spraying in the HPCI room, on October 21, 2021.

Licensed Operator Requalification Training/Examinations (IP Section 03.02)===

(1) The inspectors observed and evaluated License Operator Requalification Program (LORP) Scenario H-LTC-SE-00183, "RCIC Torus Sensor/Lost of All ISO Phase Cooling/Recirculation Master Push Buttons Failure/LLS SRV Fails Open/LOSP with EDGs Failing to Start/Loss RCIC Suction/ 2A EDG Restoration," for two teams during annual simulator evaluations on October 5, 2021.

71111.12 - Maintenance Effectiveness

The inspectors evaluated the effectiveness of maintenance to ensure the following structures, systems, and components (SSCs) remain capable of performing their intended function:

(1) Review of the Unit 1 and 2 process radiation monitoring system D11 for functional failures on November 2, 2021.
(2) Review of the Unit 1 and 2 direct current (DC) system R42 for functional failures on November 9, 2021.

Quality Control (IP Section 03.02) (1 Sample)

The inspectors evaluated the effectiveness of maintenance and quality control activities to ensure the following SSC remains capable of performing its intended function:

(1) Unit 2 "A" emergency diesel generator during a system outage, on December 8, 2021.

71111.13 - Maintenance Risk Assessments and Emergent Work Control

Risk Assessment and Management Sample (IP Section 03.01) (5 Samples)

The inspectors evaluated the accuracy and completeness of risk assessments for the following planned and emergent work activities to ensure configuration changes and appropriate work controls were addressed:

(1) Unit 2 elevated risk due to the failure of the '2A' EDG output breaker to close during testing resulted in an inoperable 2A EDG, on October 4, 2021.
(2) Unit 1 elevated risk due to the RHR and RHRSW division '1' system outage, on October 7, 2021.
(3) Unit 1 elevated risk due to the HPCI system outage, on October 19, 2021.
(4) Unit 2 elevated risk due to the RHR and RHRSW division '1' system outage, October 26, 2021.
(5) Unit 2 elevated risk due to "A" adjustable speed drive repair and single loop operations, on December 12, 2021.

The inspectors evaluated the licensee's justifications and actions associated with the following operability determinations and functionality assessments:

(1) CR10832023, Unit '2A' EDG output breaker failed to close during monthly operability surveillance on October 4, 2021.
(2) CR10836527, Unit 1 HPCI oil leaking from hydraulic manual turbine trip and reset button and from turbine driven shaft on October 23, 2021.
(3) CR10846881, Unit 1 'D' residual heat removal service water pump missing seismic restraint on December 14, 2021.

The inspectors evaluated the following temporary or permanent modifications:

(1) Temporary modification to install a temporary diesel driven fire pump due to the failure of the installed diesel driven fire pump. A new diesel driven fire pump is being procured for installation.

The inspectors evaluated the following post-maintenance test activities to verify system operability and functionality:

(1) Per Unit 2, 34SV-R43-001-2, "Diesel Generator 2A Monthly Test," Version 30.1, following replacement of damaged cable for synchro switch on October 5, 2021.
(2) Per Unit 1, 34SV-T48-001-2, Unit 2, "Reactor Building To Suppression Chamber Vacuum Relief System Operability," Version 4.13, following the adjustment of a valve's limit switches after failure on the initial operability test on October 17, 2021.
(3) Per Unit 1, 34SV-E41-002-1, "HPCI Pump Operability," Version 33.2, following the main pumps inboard mechanical seal replacement, on October 25, 2021.

71111.20 - Refueling and Other Outage Activities

(1) The inspectors evaluated the licensees response to a Unit 1 forced outage that was initiated by licensed operators inserting a manual scram due to unexpected main steam line pressure oscillations. This inspection will be completed in the first quarter of 2022.

71114.01 - Exercise Evaluation

Inspection Review (IP Section 02.01-02.11) (1 Sample)

(1) The inspectors evaluated the biennial emergency plan exercise during the week of November 1, 2021. The exercise scenario simulated a tornado strike in the electrical switchyard, which caused the plant to experience cascading failures (e.g., the swing emergency diesel generator (1B EDG) failed to start & the 2A EDG started but the breaker failed to close), culminating in a loss of offsite power on Unit 2 and meeting the conditions for declaration of an Alert. A short time later, the 2C EDG began to experience increased vibrations to the point of catastrophic failure, meeting the conditions for declaration of a Site Area Emergency. Lastly, since restoration of at least one alternating current (AC) emergency bus in less than 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> was not likely, the conditions for declaration of a General Emergency were met and allowed the Offsite Response Organizations to demonstrate their ability to implement emergency actions.
