The items under Section II above were discussed in detail. The licensee concurred that the required actions were not complete and that the items remain open.

The inspector stated that during his review of MWK and B&W pipe weld radiographs and radiographic procedures, he found an area of concern involving conformance with the intent of Code Case 72 and the ASME Code,Section III.

The licensee agreed to examine the radi.ographic requirements concerning large diameter pipe and the routine use of film side penetrameters.

The ' inspector reviewed the actions taken on this previous violation (73-1/A1). The licensee has reviewed.730 completed field weld data

'

. The results of this review showed that 19 welds were cogleted-sheets.

using 1/8-inch diameter weld filler metal for part of the weld which was not called out for by weld procedure SS-1-TIG.

In addition, 332 welds were welded to the same procedure, SS-1-TIG, without the use of

'1/16-inch diameter weld filler metal as required by the procedure.

The 19 weldments made with 1/8-inch diameter filler metal are being 100% radiographically inspected'to provide evidence that the welds are acceptable. Thirty-two welds which were welded without the prescribed 1/16-inch diameter filler metal have been -

radiographed. The radi_ographic inspection, together with the required evaluation, is not yet coglete. All other actions (it2ms 2 through C)

stated in the licensee's letter to Region II, dated March 23, 1973, I

are co g leted. The licensee agreed that the required actions on this violation are not complete and the item remains open.

Decay Heat and Makeup and Purification Sytems Radiography The inspector reviewed the action taken on this previous violation (73-1/A2). The licensee has recalled all film for review of pene-trameter location and has instructed the radiographers to place the penetrameters in accordance with the prescribed procedure. The licensee is presently cogiling a list of all film that does not

meet the penetrameter location requirement. This item remains

open until the list is cog iled and the evaluation complete.

Radiography of Piping The inspector reviewed the radiographs on makeup welds 260, 241, 261 and feedwater weld 251. These are prefabricated spool pieces and welds, fabricated and radiographed by MWK. - The radiographic technique used on the above welds was a contact shot (burning out one wall) with a film side penetrameter. MWK radiographic procedure, i

ES-403, was also reviewed. - Paragraph 4.1.1 of the procedure states

'

that the penetrameter shall be placed on the side nearest the radiation source but that where it is physically impossible to do this, a film side penetrameter shall be placed on the film. side of the joint. Moreover, the procedure states that all penetrameters -

shall be in accordance with B31.7, code Case 72, Table 1.

Nondestructive Examination Instruction (9T-lE-200) are in accordance with ASME, B&PV Code,Section III (1971). This procedure used Table IX-3325.2 for penetrameter requirements for all weldments, regardless of

source side and film side-placement. This table is identical to the

Code Case 72, Table 1, which allows 4T sensitivity up to one-inch single wall thickness range. - The licensee will examine this procedure to ensure' all necessary requirements are met.

I In conclusion, the licensee agreed to investigate the acceptability j

of MWK Radiographic Procedure ES-403, B&W Radiographic Procedure 9T-102A, and the radiographs in accordance with Code Case 72's intent and wording and ASME B&PV Code,Section III. This item-is

Missing Radiographs J

During the last inspection, radiographs for weld 42-DH could not be located even though this weld had been completed and accepted.

The radiographs were located and reviewed by the inspector with

no discrepancies found. The inspector considers this ~ item resolved.

Suspect Lack of Material Control - Weld Filler Material The licensee has received the technical report from B&W that Crystal River 3 steam, generator contains some weld filler material whidt did not indicate sufficient tensile or impact properties.

Quality Control Procedures The inspector reviewed the B&W Quality Control Manual, NSS-7, which is used exclusively at Crystal River 3 by B&W.

This manual is applicable for, but not limited to, primary, makeup, decay heat, core flood, pressurizer spray piping systems and associated valves.

The quality control procedures reviewed were:

" Procedure for Maintenance of Performance Qualification" 9WP-100N, " General Procedure for Manual Metal Arc Welding" 9WP-200N, " General Procedure for Manual Gas Tungsten Arc Welding" 9T-102A,

" General Procedure for Radiographic Examination" 9T-102H,

" General Procedure for Liquid Penetrant Lxamination" 9T-lO2F,

" General Procedure for Magnetic Particle Examination" 9A-140,

" Weld "ontrol Record" B&W field construction procedure No. B&W 037 for the erection of stainless steel pipe and associated valves, procedures for the qualification on NDP personnel and procedures for heat treating were also reviewed.

These procedures are being revised as a need is determined. The inspector stated that he had no further questions at this time except in the radiographic procedure as mentioned in paragraph 4 above.
