Initiation Reactor Isolations Following a Manual

This LER documented the initiation of engineered safety features during a
,
normal shutdown from power. On September 11, 1992, during a normal shutdown
from power, the reactor was scrammed at 22 percent power. During the water i
level drop inside the reactor associated with the scram, water level fell
below the low level setpoint of 4.5 inches, causing the following systems to l
isolate; primary containment, reactor water cleanup, and secondary
containment.
The licensee determined that the root cause of the event was an inadequate
!
I procedure for controlling the alignment of the feedwater pump and valves i
during a noimal shutdown of the plant. The procedure failed to specify the
!
alignment to be used to raise water level prior to initiating a manual scram.
The licensee's corrective actions include:
I Procedure 2.1.4, " Normal Shutdown From Power," and 2.2.28.1, "Feedwater
"
e System Operator," were revised to provide improved instructions for system alignments and operational guidance in support of a manual scram
,
at various power levels during shutdown operations.

Training for licensed control room personnel was provided which reviewed i
,
operating procedure revisions to provide better guidance for operator
!
control of vessel level during a controlled plant scram / shutdown.
'
The inspector reviewed documentation for the completion of the corrective i
actions taken by the. licensee. Based on the review performed by the l
inspection. it appeared the licensee had taken appropriate actions to preclude
'
repetition of this event.
