Results of Containment Building Tendon Tensioning i

The prestress subcontrator's final report was submitted October 16, 1973.

It contains a summary of individual vertical, dome and horizontal tendons in containment building for Unit 1.

The reasons for discrepancies in elongation in the tendons have been analyzed as required by ACI 318.

l The final review concludes that loss of prestress due to broken or out-j of-tolerance wires is below the value of one-half (1/2) percent as i

allowed in specifications.

The constructor, the A-E and the licensee j

have concurred in the corrective action, disposition and have signed off all deviation reports on tendon tensioning.

3.

Control of Welding Material Used in Containment Liner Plate Welding Procedure WRC-ll, Revision 3, of the containment liner plate contractor, Chicago Bridge and Iron Company (IB&I) dated 11/11/69 requires that records of all weld rod received and verified mill certificates be kept at the job site.

The RO inspector's review found good trace-ability without deficiency in these records. The CB&I procedure additionally states that supervisory personnel and quality control personnel only will have access to electrode storage and be re-sponsible for issuing electrodes.

CB&I's welding supervisor is responsible for issue and temperature control of E-7018 electrodes for selection of the proper rod and control of weld rods in the work area.

Documentation of these requirements at the time the containment liner plate welding was presented were not required by specifications nor by the CB&I procedure. However, the RO inspector verified by surveillance and audit reports that the requirements of the CB&I procedures were met

Control of Welding Materici Used in Reactor Bleed Tanks

p Procedure DB 119-189, Revision dated 3/20/69, controlled the work on the g-)

reactor coolant bleed tanks fabricated at the site by the Pittsburgh-Des

Moines Steel Company (PDM).

The procedure requires that electrodes and s.

filler metal conforming to specific requirements be purchased in hermetical-

!

ly sealed containers from an approved supplier.

Procedural controls are established for issuance from containers, time out of container, holding oven temperature, and rebake temperature when electrodes are not dis-carded.

The RO inspector reviewed evidence that the specified electrodes were cbtained from an approved supplier, and that receiving inspection reports included chemical and physical test cerrifications for each order.

The job specifications and the PDM procedures in force at the time did not require that records be kept by PDM covering electrode issuance and field

control.

However, the RO inspector verified in audit reports by others that after some corrective action was taken, the specification and procedural re-quirements controlling the use of welding material were satisfactorily implemented, and no further deficiencies were identified.
