INITIAL SYSTEMATIC ASSESSMENT OF LICENSEE PERFORMANCE (SALP) REPORT This forwards the initial SALP report (50-285/93-99) for the Fort Calhoun Station. The SALP Board met on February 26, 1993, to evaluate Fort Calhoun's performance for the period of August 1,1991, through January 30, 1993.
The performance analyses and resulting evaluations are documented in the enclosed initial SALP report.
In accordance with NRC policy, I have reviewed the SALP Board's assessment and
-
concur with the ratings. Good performance in the areas of Plant Operations and Maintenance / Surveillance resulted in a Category 2 rating.
Good performance in the Radiological Controls functional area resulted in a Category 2 rating, but this represents a decline from the superior level of performance that was observed during the previous assessment period. Superior performance in the Emergency Preparedness functional area resulted in an increase to a Category I rating.
Continued superior performance was noted in the areas of Security, Engineering / Technical Support, and Safety Assessment / Quality Verification, which were rated as Category 1.
Overall, licensee performance was good.
It was apparent that management's priority was the safe operation of the facility.
However, the plant perturbations caused by personnel errors and the loss of shutdown cooling indicated a need for increased management attention in these areas.
In addition, weaknesses in the radiological protection program were evident due to the two uptake events that occurred during this assessment period.
On the basis of the SALP Board's assessment, the length of the SALP period uill be approximately 15 months. Accordingly, the next SALP period will be

On April 30, 1993, Omaha Public Power District (0 PPD) met with NRC management to discuss the results of the Systematic Assessment of Licensee Performance (SALP)
j report for Fort Calhoun Station.
At that time, OPPD provided verbal comments on
the SALP Report recomendations.
OPPD will continue to proactively implement enhancements in the various SALP functional areas by reinforcing attention to detail, strengthening outage control and maintaining strong management oversight.
OPPD has prepared written responses to the three SALP report recommendations and

Improve controls over outage management and address the occasional l
lack of attention by operations personnel, j

OPPD Response:
In early fall 1992, OPPD initiated the Nuclear Process Management f
Team (NPMT) with a specific goal to improve the conduct of refueling outages.
This effort is supported by all managers and i
focuses on the outage planning and implementation processes. Eight i
I Outage Performance Enhancement Teams (OPETs) and 16 High Impact Teams (HIT) have been sponsored by the NPMT focusing on specific
,
outage processes.
Appropriate recomendations from this effort i
will be in place prior to the 1993 outage.

During outages there will be Senior Reactor Operators (SR0s) in the.

compliance with Limiting Conditions for' Operation (LCOs) and perform administrative functions to assist the Shift Supervisor.

An SRO Safety Advisor will provide continuous independent oversight i
to the Shift Supervisor during the outage and make recommendations
)
as necessary.
-l The need for attention to detail by operations personnel is
!
constantly reinforced by management through regular weekly meetings l
with the training crews. The Division Manager - Nuclear Operations
,
'
routinely has meetings during requalification training cycles with eacn crew to empnasize the need for self-checking and to enhance awareness of various operational issues.
In addition, attention to detail continues to be emphasized at various plant meetings including periodic Shift Supervisors'
meetings, Maintenance meetings and monthly Safety meetings.
OPPD also continues to utilize the ongoing training process to reinforce attention to detail.
Currently there are seven Shift Supervisors, six of which are on shift while the other Shift Supervisor is responsible for personnel l
performance issues. Areas covered include looking anead to prevent l
incidents, how incidents happen, and general corrective actions to prevent recurrence. These issues are then discussea at quarterly meetings with all operating crews during their requalification training cycles.

NRC Recommendation:
Management snould accress filling of the vacant positions in the radiological protection department, resolve the performance of i
effluent monitors, anc resolve the causes for the high numbers of contaminations.
OPPD Response:
As of April 1,1993, the position of Supervisor - Radioactive Waste has been filled. Interviews have been conducted for the Supervisor
- Radiological Health & Engineering position.
This position is currently seneduled to be filled by June 1, 1993.
In early 1992, OPPD began the design portion of a major modification to completely replace the effluent monitoring system
'
and construction is currently in progress. This modification will be completea during the 1993 Refueling Outage.
Regarding the high numoer of contaminations, OPPD has established a High Imcact Team to review and formulate actions to reduce personnei contaminations.
Recommenaations from the team will be implementec prior to the 1993 Refueling Outage.
Maintenance / Surveillance NRC Recommendation:
Management should perform an assessment of the effect that nonsafety-related maintenance activities could have on safety-related systems and provide an appropriate level of oversight of
,
surveillance testing during refueling outages.
OPPD Response:
In Septemoer 1992, CPPD initiated several actions to evaluate and correct ecuipment reliability issues, one of wnich was the establishment of the Plant Reliability Task Force.
One of the goals of this task force was to heignten awareness and aggressively pursue reliability issues. One recommendation of this task force was to concuct weekly reliability meetings which are ongoing.
Operations is deveiocing a
new procedure to govern what controls / evaluations will be needed for voluntary LC0 entry and removal cf ecuipment from service that affects reliability. Until the procecure is issued, the Plant Manager is informed of entries into LCOs with time limits of 7 days or less and is required to be apprised of any reliability issues.
Since its issuance in early 1992, CPPD has been emphasizing better
use of Stanaing Order G-87 "Non-routine Activities Requiring Formalizea Plans".

OPPD currently nas various programs in place to address reliability issues and will review the acequacy of these existing programs (i.e., 50-G-87, caily planning meetings, Reliability Task Force)
with regard to addressing the effect of nonsafety-related maintenance activities on safety-related systems. This review will be completed within the next 90 days.
Concerning the level of oversight of surveillance testing during the refueling outage, OPPD plans to have a Shift SR0 on each outage shift to facilitate work including coordination of surveillance testing system alignments.
The SRO in the OCC will ensure compliance with LCOs and perform administrative functions to assist the Shift Supervisor. As previously noted, an SRO Safety Advisor

will provide continuous independent supcort to the Shift Supervisor I
during the outage and make recommendations as necessary.
