Licensee Action on Previously Identified Items (0 pen) Unresolved Item (306/78-16-01):
Item a.
No procedural requirement for documenting snubber leak locations, and Item b.
No provisions for measuring the cold position setting (CPS) and hot position setting (HPS) in accordance with design.
In regard to item a, the inspector reviewed Test Procedures No.
2171, Rev. 2, dated March 2, 1979 and No. 1171, Rev. 1, dated March 2, 1979, both entitled " Technical Specification Shock Suppres-sor Check," and considered them adequate.
In regard to item b, the licensee stated that CPS's and HPS's had been measured during hot functional testings, and records will be made available during the inspector's next visit.

Dismantling, Repair Storage and Installation," dated April 23, 1979, and coasidered the preventive measures adequate.
(Closed) Unresolved Item (306/78-16-09):
Any leak on B-E horizon-tally installed snubber cylinder wall, the vacuum may be able to draw air into the unit. During the December 20, 1978, meeting the licensee stated that their present snubber surveillance program requires all leaking snubbers be removed for seal replacement, examination and testing.
The inspector reviewed Test Procedures No.
1171, Rev 2, dated May 4, 1979 and No. 2171, Rev. 3, dated May 4, 1979 " Technical Specification Shock Suppressor Check," and considered the item closed.

Snubber Concrete Expansion Type Anchor Bolts Some installation drawings prepared by Basis-Engineers a.
(B-E) were reviewed by the inspector. These are:
Mk. No. MSH-62 Mk. No. MSH-64 Mk. No. MSH-65 Mk. No. MSH-66 Mk. No. MSH-67 Mk. No. MSH-68 The inspector stated that the bolts were installed too close, i.e. less than 10 diameters center to center dis-tance for the 3/4" and 7/8" bolts specified, that the shear cone developed under load will probably be over-lapping each other and cause reduction in load capacity.

Inspection of snubber installations at the reactor coolant pump No. 12 area at least six concrete anchor bolts were observed being installed not perpendicular with the concrete surface.
The inspector stated future pull tests based on recent IE Bulletin No. 79-02 could resolve the above concerns. This is an unresolved item.

Qualification Tests for B-E Snubbers A number of items relative to B-E snubber qualificatioas were discussed in RIII Report 50-306/78-16. The licensee stated that representative snubber units are to be shipped to Wyle Laboratories, Huntsville, Alabama for dynamic and environaeatal testings starting May 7, 1979.

The inspector reviewed the draft copy of the test procedures, and the Wyle Laboratories letter to NSP, " Snubber Qualification Testing," dated January 23, 1979, an< had no adverse comments.
No items of noncompliance or deviations were identified.

Replacement of Anker-Holth (AH) Snubbers A number of items relative to AH snubbers were discussed in RIII Report 50-306/78-16.
Subsequently, the licensee informed RIII that all AH snubbers at the site will be replaced by January 1, 1980 if material delivery permits. The inspector reviewed NSP Purchase Requisition (PR) to Nutech dated April 26, 1979, requesting Nutech to provide purchase specification for replacement snubbers for AH's of the 90K and 6K size by June 1, 1979. The inspector questioned why NSP took so long to write the PR.
The licensee stated that they are sure that replace-ments will be installed prior to January 1, 1980.
No items of noncompliance or deviations were identified.

Observation of Snubbers and Test Machine The inspector observed the four large hydraulic snubbers installed on steam generator No. 12, and observed approximately 10 small B-E snubbers in the reactor coolant pump No. 12 area.
No adverse snubber conditions were identified.
The inspector also observed the newly installed snubber functional test machine manufactured by Bergen-Patterson Corporation.
The licensee stated that the machine had not been completely set-up, but it will be in operational status in the near future.
No items of noncompliance or deviations mere identified as a result of the observation.

k Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance, or deviations. One unresolved icem dis-closed during this inspection is discussed in Paragraph I of this report.
Exit Interview The inspector met with site representatives (denoted in the Persons Contacted paragraph) at the conclusion of the inspection on.iay 4, 1979. The inspector summarized the scope and findings of the inspection noted in this report.
