11 QA Investigation of Orientation Changes on All T~ermocouples Installed on Safety Injection Lines and Pressurizer Surge Line" In an effort to evaluate the. thermal gradient concerns raised in IE Bulletin Nos. 88-08 and 88-11, the licensee initiated the placement of thermocouples as temporary instrumentation to record temperature oscillation They are intended to serve as an interim measure pending a long term alternativ Installation of the thermocouples
and related instrumentation was completed and signed off on December 21, 1988 according to licensee documentatio In February 1989, an NRC inspector walked down the affected Unit 1 systems to verify the installation of the thermocouples and associated wiring and instrumentatio Numerous discreP.ancies were noted, prompting the issuance of the two unresolved item The Quality Control {QC)
inspector who signed off on the work speculated that the discrepancies could be the result of craft workers working or walking around the instrumentation or reinstallation of the pipe insulatio The licensee issued Engineering Work Requests (EWRs)88-425 0, dated March 21, 1989, and 88-448 N, dated March 9, 1989, for Units 1 and 29 respectivel They were issued to ensure proper installation via a final Engineering and Quality Control walkdown to inspect and reorient out-of-position thermocouples as well as to replace those which were found to be damage To assure that they would not be disturbed by craft workers, the walkdown was conducted as the final activity before closing the area in preparation for start u The Unit 1 walkdown was completed and signed off by QC on June 9, 198 The Unit 2 walkdown was completed and signed off by QC on July 21, 198 The steps taken by the licensee appear to be reasonable and adequate to assure proper thermocouple installation. "Pipe Support Calculation Review and for Unit 1
During Inspection No. 88-20, the NRC inspector randomly selected seven piping isometrics which.included 36 hangers {pipe hangers and component hangers) which had had final QC inspectio The isometrics included portions of the Component Cooling Water System, Safety Injection Loops, and Auxiliary Feedwater Syste In addition to reinspecting the piping against the engineering drawings for such things as configuration, branch locations, hanger locations, and clearance with adjacent structures and/or piping, the inspector reinspected the hangers against their engineering drawings for such things as configuration, member size, welding, base plates, and fastener/anchor bolt installatio Also, several design calculations for Unit 2 supports were reviewed and evaluated for thoroughness, clarity, consistency, and accurac They were deemed to be of good qualit However, the design calculations, for Unit 1 supports were not reviewed at that tim During the current inspection, the inspector reviewed 19 Unit 1 support calculations for content, clarity, and accurac Specifically, the following were reviewed:

The calculations appeared _to be professionally done, utilizing applicable codes and standards, reasonable assumptions, and sound engineering judgmen Therefore, IFI 50-280/88-20-05 is hereby close (2)

IFI 50-281/88-51-03:
"Evaluation of Whip Restraints on Pressurizer Surge Line" One of the principal reasons for conducting this investigation was to assist the Resident Inspectors with their evaluation of this issu The Regional Inspector reviewed the situation, including looking at appropriate drawings to clearly understand the physical configuration, and interviewing engineers who participated in the original walkdown for any details which may have been unnoticed previousl The findings of the Regional Inspector were summarized and g.iven to the Resident Inspector who will officially address this IFI in a future monthly repor.
New Open Items Pipe and Support Calculation No. 14937.53-NZ(B)-005-ZB-007, Revision 0, entitled, "Re-analysis of Support H-34," dated June 24, 1988 did not properly analyze the suppor Specifically, the* members used in the support have unsymmetrical cross sections (structural angles were used)
but the calculations neglected to analyze them as such, thereby neglecting the effect of, loading-induced torsional stresses. This torsional behavior wi 11 a 1 so trans fer a tensile force into the anchor bolts in this cas This was also neglected in the analysi Engineering personnel of the 1 i censee I s Archi tectura 1 /Engineering firm agreed with the inspector's findings and committed to revising the calculations by month's end. Therefore, IFI 50-280/89-29-01, "Re-Analysis of Pipe Support to Account for Unsymmetric Members," is hereby opened to track this issu While in this particular case, it is expected that the re-analysis will still find the support to be adequate, greater assurance is needed that other pipe supports using members of unsymmetric cross section have been properly analyzed, including the torsional effect Otherwise, reliability and operability questions may be raise Therefore, the licensee is directed to conduct a survey of calculations of supports which include unsymmetrical members in their configuration to determine if the analysis has properly accounted for torsional effects. Until the results of the survey are reviewed, UNR 50-280,281/89-29-02, "Survey of Calculations of Supports with Unsymmetrical Members,".is hereby opene.
