.s . On May 24, 1982, NRC Region V issued a Severity Level V Notice '

of Violation (" Notice"), as part of NRC Inspection Report 50-275/82-13, on Diablo Canyon Unit 1. The Notice cited a concern related to s

construction wcrk performed by PGandE's construction contesctor, Pullman Power Products (" Pullman"). The violation was described in the Notice as: x

Products on February 27, 1981 identified a condition where a required pre-hydrostatic L

test walkdown had not been performed, and -

' On April 22, 1982, arc strikes were found on , piping associated with four previously -c performed hydrostatic tests. In addition, , tape residue was found on piping associated - with two of these tests. The

. The concern originated from an internal audit performed by
,
,

Pullman on February 27, 1981. At that time, Pullman discovered that certain pre-hydrostatic test documentation was signed by the Quality Engineer (QE) Leadman, instead of the Quality Control (QC) Field w Inspector as required by current procedures. W e corrective action taken by Pullman at the time was to instruct both the QE Leadman and the QC Inspectors that all such tests must be conducted and documented by the QC m_ Inspectors. W e !GC, based on indications (tape residues / arc strikes) E found during its April 22, 1982 inspection, determined that Pullman's corrective actions failed to address past work since they did not identifyandcorrecttheseindications.

Subsequent to the imC inspection, Pullman took additional actions to address the past practice regarding pre-hydrostatic tests.

These actions consisted of Pullman confirming with both the QE Leadman and the QC Inspectors involved in the previous tests that the QE Leadman's signatures on the test documentation were based on proper and

satisfactory completion of the pre-hydrostatic test walkdowns by the responsible QC Inspectors. Additionally, Pullman determined that the previous test walkdowns demonstrated the acceptability of the areas

included in these walkdowns. PGandC notes, however, that it is not always possible to detect indications 1/64-inch in size or smaller during a routine pre-hydrostatic walkdown. In addition, PGande believes that, due to the type and nature of the indications noted above, the length of ' time since the completion of the test walkdowns, and the amount of construction constantly being perfomed in the areas where the indications were found, these indications could have been caused by events subsequent to the walkdowns. Ilevertheless, both PGandE and Pullman have evaluated the indications found by the imC and have taken

, appropriate steps to have them renoved.

Based on the actions described above, as well as actions previously taken by Pullman, PGandE believes that adequate corrective action has now been taken and full compliance has been achieved. Therefore, no further

,

action is necessary on this Ibtice.
