Operational Safety Verification (71707)

The inspectors evaluated licensee activities to confirm that the facility was being operated safely and in conformance with regulatory requirement These activities were confirmed by direct observation, facility tours, interviews and discussions with -licensee personnel and management, verification of safety system status, and review of facility record To verify equipment operability and compliance with TS, the inspectors reviewed shift logs, Operation's records, data sheets, instrument traces, and records of equipment malfunction Through work observations and discussions with Operations Staff members, the inspectors verified the staff was knowledgeable of plant conditions, responded properly to alarms, adhered to procedures and applicable administrative controls, cognizant of in-progress surveillance and maintenance activities, and aware of inoperable equipment statu The inspectors performed channel verifica tions and reviewed component status and safety-related parameters to verify conformance with T Shift changes were routinely observed, verifying that system status continuity was maintained and that proper control room staffing existed. Access to the control room was controlled and operations personnel carried out their assigned duties in an effective manne Control room demeanor and communications continued to be effectiv Plant tours and perimeter walkdowns were conducted to verify equipment operability, assess the general condition of plant equipment, and to verify that radiological controls, fire protection controls, physical protection controls, and equipment tagging procedures were properly implemente Facility Tours During a routine system walkdown on April 3, 1991, the inspectors observed that there was not SW flow through the A SI pump bearing oil coole The deficiency was reported to Operations who immediately took action to correct the condition. The SW supply valve to the bearing cooler, SW-513, was found throttled open at least one half turn as required by operating procedure, OP-903, Service Water Syste The valve was cycled open to flush the line and valve seat. The valve was subsequently rethrottled to establish adequate flow through the bearing coole Evidently, some debris had plugged the line and/or valve. Subsequent inspections have not revealed a similar occurrence on either operable SI pum On April 15, 1991, the inspectors observed that V2-6A, feedwater isolation valve to A S/G, had a galled ste The licensee indicated that this condition had previously been identified and evaluated as being acceptabl However, leaving the stem in the observed condition for an indefinite

period is not prudent. The inspectors were unable to locate an initiated work request to correct the conditio At the end of the report period, the licensee was reviewing this situatio On May 1, 1991, the inspectors accompanied licensee personnel during a routine monthly at power CV entr Conditions were not observed which would render safety-related equipment inoperable. However, safety-related instrumentation, LT-920, LT-930, FT-416, FT-424, FT-425, and FT-426 (SI accumulator level and RCS loop flow instruments) were observed to have boric acid build up on tubing connection The licensee initiated work requests to correct these condition Inadvertent TSC/EOF/PAP Diesel Generator Trip On April 13, 1991, during performance of OST-405, TSC/EOR/PAP Diesel Generator (semi-annual), the TSC/EOF/PAP diesel generator was tripped when an operator inadvertently opened the excitor breaker while attempting to silence an AC-DC alar The diesel generator was promptly returned to service. The licensee initiated ACR 91-134 to determine the root cause(s)

The inspectors observed certain safety-related surveillance activities on systems and components to ascertain that these activities were conducted in accordance with license requirement For the surveillance test procedures listed below, the inspectors determined that precautions and LCOs were adhered to, the required administrative approvals and tagouts were obtained prior to test initiation, testing was accomplished by qualified personnel in accordance with an approved test procedure, test instrumentation was properly calibrated, the tests were completed at the required frequency, and that the tests conformed to TS requirements. Upon test completion, the inspectors verified the recorded test data was complete, accurate, and met TS requirements; test discrepancies were properly documented and rectified; and that the systems were properly returned to servic Specifically, the inspectors witnessed/reviewed portions of the following test activities:

On May 9, 1991, the resident inspectors and the regional Section Chief responsible for the site met with the following Darlington County officials:

County Administrator, Emergency Preparedness Director, General Services Director, Fire District Coordinator, Public Information Officer/911 Project Coordinator, and Emergency Medical Services Operations Manager. During the meeting, the resident inspectors were introduced, the mission and role of the NRC as it pertains to the site were discussed, and an opportunity was provided for discussion of community concerns related to the operation of the facility. On May 10, 1991, the Section Chief and Senior Resident Inspector participated in a similar meeting with the Mayor and City Manager of Hartsville, S.C. The local officials in both meetings stated that their constituents had not expressed any concerns involving the safe operation of the facilit. Exit Interview (30703)
