Implementation of NRC Bulletin 88-10:
Nonconforming Molded Case Circuit Breakers The 'inspector reviewed.the licensee's implementation of NRC Bulletin 88-10
"Nonconforming Molded Case Circuit Breakers." The licensee has physically inspected all molded case circuit breakers (MCCB) in stock and attempted to establish traceability for circuit breakers purchased subsequent to August 1, 198 Of the 132 MCCBs in the plant warehouse that had been purchased as safety-related, one ITE type EQ-P breaker was determined to be untraceable and had been removed from the storeroo The licensee's physical inspection and review of procurement documentation identified seven molded case circuit breakers purchased as commercial grade which they were not able to trace to the circuit breaker manufacturer Two of these breakers had been provided to the licensee's vendors by rebuild shops identified in NRC Information Notice (IN) 88-4 The seven untraceable circuit breakers have been removed from the materials warehouse and are to be scrapped if the licensee cannot establish traceabilit *
The inspector examined molded case circuit breakers in the licensee's warehous Breakers included in the examination were manufactured by General Electric, Westinghouse, Square D and Telemechanique/IT None of the breakers exhibited external indications of tampering or refurbishmen Circuit breakers that the licensee had determined to be untraceable had been removed from the warehouse prior to the inspection.
The inspector reviewed procurement documentation for molded case circuit breakers purchased by the licensee for the Palisades plan Purchase
orders selected for review included purchases for both safety-related and commercial grade breakers. Except for the eight breakers identified as not traceable by the licensee, the documentation provided reasonable assurance that the breakers maintained as stock were traceable to the circuit breaker manufacture The inspector's review of procurement documentation for circuit breakers that had been issued from the warehouse prior to the issuance of Bulletin 88-10 identified one purchase order, PO 2003-2124 to Westinghouse Electric Supply Company {WESCo) for three Westinghouse model HKA 2200T trip unit WESCo had purchased the trip units from General Magnetics/Electric Wholesale for direct shipment from General Magnetics to Palisade General Magnetics was identified as a supplier of rebuilt molded case circuit breakers in NRC IN 88-46.Bulletin 88-10 does not require the licensee to establish traceability for MCCBs installed or issued from stock prior to August 1, 1988, if the MCCB storeroom population is greater than 50 MCCBs and traceability is greater than 80 percent. Since the HKA 2200T units had been issued prior to August 1, 1988, and the storeroom MCCB population was greater than 50 with more than 89% traceability, the purchase order had not been reviewed by the license The HKA 2200T trip units had not been installed in the plant. Prior to installation, the electrical maintenance department had installed the trip units in Westinghouse KA 2225 breakers and tested two of the unit Both units had failed the required tests and all three units had been rejected. The trip units were returned to Material Services to be scrapped while the inspector was onsite. Since the units were "new",
no root cause analysis was performed, nor were the failures reported pursuant to 10 CFR 21 since the units were processed commercial grade and being upgraded for use in control rod drive clutch circuit The inspector reviewed three completed safety-related work orders that involved replacing molded case circuit breakers. The work orders required that three breakers be installed. Review of procurement documentation revealed that each of the breakers was traceable to the circuit breaker manufacture The licensee contacted each of their MCCB vendor The vendor was requested to determine whether the source for MCCBs supplied to Palisades included one or more rebuild companies from a list of 14 suspect suppliers from IN 88-46 Supplement The list did not include General Magnetics, PENCON International or Electro Components, companies identified in NRC IN 88-4 The licensee has initiated further contacts with their MCCB vendors to ascertain whether their vendors' sources included these companie Based on the reviews, examinations and inspections identified above, the inspector determined that the licensee was adequately implementing NRC Bulletin 88-1 The bulletin as applicable to Palisades, did not require the licensee to evaluate installed MCCBs purchased from vendors who supplied the plant with untraceable MCCBs and the inspector found no indication that installed MCCBs had been evaluated for traceabilit **
