Dear Mr. DelGeorge:
This refers to the region based team inspection of engineering and technical support conducted by Mr. Robert Lerch and others of this office on May 10 through 28, 1993.
The inspection included a review of authorized activities for your Dresden Nuclear Power Station. At the conclusion of the inspection, the findings were discussed with those members of your staff identified in the enclosed report.
The purpose of this inspection was to assess the quality and effectiveness of engineering involvement in plant activities.
The inspection consisted of a review of modifications to the isolation condenser and the low pressure coolant injection systems during the current Unit 2 outage; a review of active temporary alterations, work packages, and relevant procedures; and interviews with plant staff.
During this inspection, we noted several weaknesses, some of which were previously identified by your staff, involving work practices and communications.
Examples included failures to follow your own procedures; failure to identify a potential electrical overload of a power supply resulting in the need for a design change; removal of a piping support without an appropriate analysis of the effect; and miscommunication between engineering personnel and installation personnel which left workers unaware they were working with energized wires.
While individually these issues were not significant to plant *safety, collectively they point out the need for additional management attention.
Notwithstanding these concerns, we were encouraged by the positive initiatives you have recently undertaken to strengthen your engineering organization, including the addition of resources and organizational changes made in the Technical Staff.
However, it was too soon to d_etermine the impact of these changes.
During this inspection, certain of your activities appeared to be in violation of NRC requirements.
However, as described in the enclosed inspection report, the violation was categorized at Severity Level V and is not being cited because the criteria specified in Se~tion VII.B.l of the "General Statement of Policy and Procedures for NRC Enforcement Actions," (Enforcement Policy, 10 CFR Part 2, Appendix C (1992), were satisfied.
Nevertheless, similar problems were identified earlier by your Quality Verification organization which were not yet fully addressed.
