Inadequate Tracking of Requalification Training Lectures (Unit 1)

The inspector examined the new status board for tracking requalification training lectures. The inspector concluded that this status board would adequately address this previously identified concern.

Inadequate Completion of Maintenance Packages (Unit 1)

The inspector reviewed the licensee's new system for assuring completion of safety-related maintenance work packages performed by the Station Maintenance organization. The system appears to conform to Station Order S-M-103, " Maintenance Order." Four maintenance orders were examined and were found to be complete. The inspector will further verify implementation of the new system during a future inspection.

Shift Technical Advisor (Unit 1)

The inspector reviewed Engineering Procedure 501-V-5.7, Revision 0, dated October 14, 1981, "Outies, Responsibilities and Authority of the Shift Technical Advisor"; Training Memorandum 7-80, Revision 1,

" Shift Technical Advisor Training and Requalification Program";

and Station Incident Report No. 81-29. The inspector concluded that the shift technical advisor requirements as stated in NUREG-0737 appear to be satisfied.

Shift Manning (Unit 1)

The inspector examined an SCE letter to the NRC, dated February 4, 1982 (K. Baskin to D. Crutchfield), and interviewed the Unit-1 Superintendent. The inspector concluded that the licensee is attempting to meet the TMI I.A.1.3 requirements on or before November 1, 1982, but even under adverse circumstances should meet the requirements not later than February 1, 1983.

Seismic Event Monitoring and Recording (Unit 1)

The inspector examined the licensee's Seismic Event Monitoring and Recording Instrumentation Program.

Installation, maintenance, and calibration of the system is performed by Kinemetrics Incorporated.

Documentation relating to these activities is sent to the SCE Corporate Office as it is completed. The inspector reviewed the December 12, 1981 surveillance and calibration data supplied to SCE by Kinemetrics.

The inspector also reviewed Kinemetrics qualifications, personnel qualifications, NBS traceability, procedures, and Quality Assurance program. Based on this review, the inspector concluded that the licensee's program appeared to be appropriate and acceptable.

Housekeeping / Cleanliness Program (Unit 1)

The inspector examined the Station Order S-A-10, Revision 9, dated February 1, 1980, " Station Inspections and Housekeeping," and Station Order S0123-M-5, Revision 2, dated February 8, 1982, " General Maintenance Order." Based on these reviews, the inspector identified that the program does not currently address housekeeping zones, control of housekeeping during work activities and prompt removal of combustibles.

Two new procedures, S0123-A-130 'and S0123-VI-1.5, reference and address these items and all requirements in ANSI N 45.2.3-1973 and ANSI N 18.7-1976.

These new procedures require on-site committee review and approval before implementation.

Cleanliness Control is also addressed in Station Order S0123-M-5 and in specific maintenance procedures.

These procedures reference and conform to applicable standards.

The inspector verified that cleanliness controls were appropriately referenced by examining several work packages.

The inspector, accompanied by licensee management, inspected housekeeping conditions both inside and outside of containment.

During this inspection, several instances were observed to be in nonconformance with the station program.

Station management issued corrective action orders to clean up these problem areas. All work was completed while the inspector was still on-site.

No items of noncompliance or deviations were identified.

Test and Measurement Equipment Program (Unit 1)

The inspector reviewed Station Order S0123-M-12, Revision 1, dated

,

l February 8,1982, " Requirements for Calibration and Control of Measuring

and Test Equipment," and several maintenance and calibration procedures.

l The inspector also examined inventory and calibration records of electrical and mechanical test and measurement equipment.

Based on the above examination the inspector verified that the licensee's program for controlling test and measurement equipment established:

a.

Criteria and responsibilities to assure compliance with established calibration frequencies.

b.

Equipment inventory lists identifying all test and measurement equipment, calibration frequencies, calibration standards and procedures, c.

Requirements for marking each piece of test and measurement equipment with calibration status, d.

A recall system to assure calibration before the required date.

e.

Prohibitions against use of uncalibrated test and measurement equipment.

f.

A system requiring review of past calibrations performed with

]

test and measurement equipment found to be out of calibration.

The licensee connitted to adding a sign-off step to each procedure using test and measurement equipment to assure that test and measurement equipment documentation has been completed prior to closing out the job.

No items of noncompliance or deviations were identified.

7.

Independent Inspection Effort a.

Unit 1 i'

The inspector examined the work packages for the following work i

to be performed during the current extended outage by the SCE Project Organization:

(1) Work package 3.2, " Reactor Head and Pressurizer Vent System" (2) Work package 8185, " Turbine Building North and West Seismic Modifications"

(3) Work package 8159, " Reactor Coolant Pump Lube Oil Collection" (4) Westinghouse contract work package, " Steam Generator Tube Examination"

The inspector determined that these safety-related work packages contained work assignment responsibilities; safety analyses; drawing status, material status, and equipment control documentation; cleaning requirements; retest requirements; QA/QC acceptance; and return to station approval.

The work packages reviewed also complied with station procedures and checklists.

-6-The inspector examined the job locations referenced by the above work packages for evidence of compliance with the work package requirements.

Unit 2 i

The inspector examined the licensee's preliminary assessment and immediate corrective actions related to the following operating events:

'

(1) Reactor coolant system boron dilution.

(2) Loss of shutdown cooling.

(3) Dropping of a stainless steel rule inside the reactor vessel with the core loaded and vessel head in place.

The inspector found that the licensee was developing tests and

,

i experiments to determine and eliminate the causes of the first two events before continuing with the startup program. The third event was traced to inadequate tool control for the existing plant condition.

Temporary change notices were issued to correct the immediate problem. The long term solution will be to revise procedures to emphasize tool control whenever the reactor coolant system is open.

Subsequent to the third event, the licensee located the dropped rule using a borescope, removed the reactor vessel head and retrieved the rule.

At the close of the inspection period, the first two events were still being studied.

No items of noncompliance or deviation were identified.

8.

Exit Meeting The inspector met with the licensee representatives (denoted in Paragraph 1)

on March 19, 1982. The results of the inspection were discussed and the licensee committed to change test and measurement equipment procedures to effect more positive control as described in paragraph 6.
