Concrete Design, Manuf'acture 'and Placement

Review of Quality Assurance Implementing Procedures The inspector reviewed the following procedures for compliance with applicable codes and installation drawings:

Review of Records and Testing Results The following records were reviewed in order to verify that the above quality procedures were being properly implemented:
(1) Batch plant inspection activities including material control, equipment calibration, mix selection, moisture control, and control of concrete placeability during transportation to job site.
(2) Construction Inspection Data Report (CIDR) for production of concrete including slump testing, air content testing and molding of concrete compressive test cylinders.
(3) National Ready Mixed Concrete Association (NRMCA) certification of Lyman Ready Mix by a registered professional civil engineer for the -State of California.
(4) Qualification (proof test) for pre-qualified mix OT-2,.
which was qualified by Osborne Laboratories, Inc. on January 20, 1981. Osborne Laboratories, Inc. was previously qualified by U.S. Testing and Bechtel.
(5) SCE quality assurance field surveillance reports C-277-81 and C-329-81 for surveillance of U.S. Testing, and C-233-81 and C-338-81:for sur'veillance of Lyman's Ready Mix Concrete.

Pre-Placement and Placement

The inspector obser'ved concrete pre-placementfactivities in the northeast section of.the turbine building.
Condition of the forms and reinforcing' steel appeared ' satisfactory, and pre-placement cleanup was in~ progress..The concrete plac'ement scheduled to take place during this inspection was postponed due to a higher than requir~ed water level under the' soil foundation upon which the concrete was Lto be placed.

Qualification of Inspection Personnel The qualification and certifications of two U.S. Testing QC inspectors and three Bechtel civil QC inspectors involved in concrete activities were reviewed by the inspector. This included one Bechtel welding QC inspector. Review of employment histories revealed that all inspectors had adequate work experience to perform their respective duties.
In addition all QC inspectors were qualified in accordance with ANSI N45.2.6.
No items of noncompliance or deviations were identified.
3.
Structural Welding Activity The inspector examined welding of a lateral structural support beam to a gusset plate in the west section of the turbine building. The field welding checklist identified the beam as diagonal brace G to H, column 13 installed to drawing number 5166422. Weld joint 430A was completed and weld joint 430B was in progress. Welding of these joints was in accordance with WPS No. P1-AT-Lh (structural). The inspector exaniined this procedure and the welding operations for compliance to AWS D1.1.
The inspector reviewed the performance qualification records for the two welders involved in welding these joints.
Both were qualified on the date the welds were made in accordance with AWS Dl.l.
The inspector examined the control, issue, return and storage of weld filler materials for compliance with procedural requirements.
All filler material located in holding ovens and in opened containers were correctly identified. The holding ovens were calibrated and operating within the required temperature range.
No_ items of noncompliance or deviations were identified.
4.
Exit Meeting The inspector met with licensee representatives (denoted in paragraph 1)
on April 2, 1982. The scope of the inspection and findings as detailed
in this report were discussed.
