| Description | Title 10 CFR Part 50, Appendix B, Criteria XVI states, in part, Measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment, nonconformances are promptly identified and corrected. Contrary to the above, although the licensee promptly identified a condition adverse to quality, the licensee failed to correct that condition. Specifically, the licensee failed to repair a 5 drop-per-minute jacket water leak on emergency diesel generator 12 jacket water flange connection on March 3, 2013, which rapidly increased to 5 gallons per minute during a surveillance run on March 11, 2014. The failure to take timely corrective actions to resolve a condition adverse to quality was a performance deficiency. The performance deficiency was more than minor because it was associated with the equipment performance attribute and affected the Mitigating Systems Cornerstone objective to ensure availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Inspection Manual Chapter 0609, Appendix A, The Significance Determination Process (SDP) For Findings at Power, dated July 1, 2012, inspectors determined that the finding was of very low safety significance (Green) because the finding did not 1) affect the design or qualification of a mitigating SSC; 2) did not represent a loss of system or function; 3) did not represent an actual loss of function of at least a single train for greater that its technical specification allowed outage time; and 4) did not represent an actual loss of function of one or more non-technical specification trains of equipment designated as a high safety-significant in accordance with the licensees Maintenance Rule program for greater than 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />. This issue was entered into the corrective action program as Condition Report 14-4762. |
