DETAILED DESCRIPTION OF EVENT On November 17, 1982, with the Plant in Mode 1, Condensate Polisher BP,ckwash water was released from the Backwash Receiving Tank QXTK-0127) to the site settling ponds without having been sampled as required by Technical Specification Surveillance Requirement 4.11.1.1.

' PROBABLE CONSEQUENCES Radiation levels for the secondary side of the Plant were non-detectable.

Also, a grab sample of XTK-0127 residue was taken af ter the event with no detectable radiation levels.

Therefore, the Licensee is assured that no adverse consequences resulted frcm the event.

CAUSE(S) 0F THE OCCURRENCE Procedures for the release of Condensate Backwash did not adgquately address the interfacing of all groups involved in the release process.

In preparing for the release, operations personnel notified the Chemistry Department per Systera Operating Procedure (SOP) 203 This was misunderstood to be the analysis for radioactivity, but actually was the analysis for pH.

The Health Physics Department performs the radioactivity analysis for releases.

IMMEDIATE CORRECTIVE ACTIONS TAKEN Af ter the event, a grab sample was taken from the Condensate Backwash Receiving Tank residue which resulted in no detectable radiation levels.

A review of all radioactive effluent procedures was initiated in order to detenmine if the proper group interfacing existed.

Comments resulting from this review are currently being evaluated for their validity.

ACTION TAKEN TO PREVENT RECURRENCE The Licensee has generated Station Order 82-004 to establish interim controls between operations and support groups to ensure proper notification for sampling prior to offsite releases.

Applicable procedures will be revised prior to January 31, 1983, as a final measure.
