LER No.82-047 Page Two December 16, 1982 DETAILED DESCRIPTION OF EVENT On November 18, 1982, with the Plant in Mode 3, operations personnel discovered that they had been in violation of Technical Specification 3 1.1.4, Surveillance Requirement 4.1.1.4.b.

The Tavg-Trer Deviation Alarm had failed and could not be reset on October 29, 1982.

The annunciator failure was subsequently repaired on November 19, 1982.

During the period of time from alarm failure until discovery of the violation, operations personnel failed to document the required 30 minute verification that Tavg was greater than or equal to 551 F while in Modes 1 and 2.

PROBABLE CONSEQUENCES No adverse consequences existed prior to the discovery of the violation because:

1.

A review of the Tavg recorder indicated that, with the exception identified in LER 82-042, Tavg did not go below 551*F while the reactor was critical.

2.

Numerous startups were made during this period of time, and in all criticalities, Tavg is routinely verified to be above 551 F.

CAUSE(S) OF THE OCCURRENCE The cause of this occurrence was operator error in failing to recognize the subject surveillance requirement for the existing

plant conditions

IMMEDIATE CORRECTIVE ACTIONS TAKEN No immediate corrective action was necessary since the Plant was in Mode 3 at the time of discovery.

ACTION TAKEN TO PREVENT RECURRENCE The operator logs were revisad on November 21, 1982, to reflect the requirement to log Tavg every 30 minutes whenever Tavg As less than 561*F and the Tavg-Trer Deviation Alarm is not reset.
