o Mr. James P. O'Reilly LER No.62-045 Page Two December 16, 1982 DETAILED DESCRIPTION OF EVENT On November 16, 1982, at 1818 hours0.021 days <br />0.505 hours <br />0.00301 weeks <br />6.91749e-4 months <br />, the Plant entered Mode 1, without performing Surveillance Test Procedure (STP) 134.001,

" Shutdown Margin Ce.iculation."

This test is required by Technical Specification Surveillance Requirement 4.1.1.1.(d),

" Reactivity Control Systems, Boration Control".

PROBABLE CONSEQUENCES All control bank measured values were within the predicted value acceptance criteria; therefore, there were no adverse consequences as a result of this event.

CAUSE(S) 0F THE OCCURRENCE The subject surveillance test was omitted in the plant startup program (due to oversight) as a prerequisite for exceeding 5%

of rated thermal power.

IMMEDIATE CORRECTIVE ACTIONS TAKEN STP-134.001 was performed satisfactorily on November 17, 1982, at 0900 hours0.0104 days <br />0.25 hours <br />0.00149 weeks <br />3.4245e-4 months <br />.

The shutdown margin was greater than the 1 77%

required per Technical Specifications.

ACTION TAKEN TO PREVENT RECURRENCE In order to prevent recurrence, the Surveillance Test Program is being revised to improve administrative controls for

" contingency type" surveillance tests (such as the performance of a Shutdown Margin Calculation prior to exceeding 5% power after each Fuel Loading).

This action will be completed by January 31, 1983.
