Mr. James P. O'Reilly LER No.82-043 Page Two December 13, 1982 DETAILED DESCRIPTION OF EVENT On November 13, 1982, with the Plant in Mode 2, the Nuclear Instrumentation Power Range Channel (N42 ) Low Range Trip Setpoint was found set at 50% icstead of the required 25% of Rated Thermal Power.

The discovery was made during the performance of Gurveillance Test Procedure (STP) 310.006, and the bistable was immediately adjusted to the proper setpoint in accordance with Technical Specification 2.2.1.a.

The Channel was not declared inoperable at time of discovery per Technical Specification 3.3 1, Table 3 3-1.2.B, since surveillance testing was in progress, and the bistable was adjusted to the correct setpoint in less than one (1) hour.

PROBABLE CONSEQUENCES There were no adverse consequences from the less conservative setpoint value since the Power Range High Setpoint had previously been adjusted to 25% of Rated Thermal Power for Power Operational Testing on October 18, 1982.

At time of occurrence, the remaining Power Range Channels were operational with the proper trip setpoints.

CAUSE(S) 0F THE OCCURRENCE The cause of this occurrence is attributed to personnel error.

During the performance of the Power Operational Testing Program, the Power Range High Setpoints are adjusted several times for the various plant power levels, and it is believed that the Low Range Bistable was set in error during these perturbations.

IMMEDIATE CORRECTIVE ACTIONS TAKEN Nuclear Instrumentation Power Range Channel (N42) Low Range Trip Setpoint was immediately adjusted to 25% of Rated Thermal Power, and all other setpoints were verified to be correct for that power level.

ACTION TAKEN TO PREVENT RECURRENCE Maintenance personnel have been instructed to exercise caution in adjusting instrument setpoints to their prescribed values. This should prevent recurrences of this nature in the future.
