DETAILED' DESCRIPTION OF EVENT On October 30, 1982, with the Plant 1n Mode 4, Source Range

~

Channel NI-32 failed low.

The channel was declared ~ inoperable-per ' Technical Specification 3.3.1, Action Statement 5.

PROBABLE CONSEQUENCES No adverse consequences resulted from this event.

Source Range Channel NI-31 was operable at time of occurrence.

CAUSE(S) 0F THE OCCURRENCE Loss of signal was due to a loose connection at DB voltage attenuator connector.

l IMMEDIATE CORRECTIVE ACTIONS TAKEN Source Range Channel 32 Level Trip was immediately placed to bypass by Operations.

The connection to the DB voltage attenuator was resoldered.

Source Range Channel NI-32 ' was returned to service af ter satisfactory performance of the l

applicable Surveillance Test Procedure _on November 1, 1982.

ACTION TAKEN TO PREVENT RECURRENCE Except for the normal surveillance requirements on the Nuclear Instrumentation Channels to demonstrate operability, _ the i

licensee plans no further action at this time.
