| Title | Failure to Verify Ventilation Design for Vital Switchgear |
| Description | Inspectors identified a noncited violation of 10 CFR Part 50, Appendix B, Criterion III, Design Control, for the licensees failure to maintain design control of the Unit 2 vital switchgear ventilation system. Specifically, in 2002, the licensee failed to ensure that the ventilation system was capable of cooling the switchgear under design basis conditions. The licensee documented the issue in Condition Report CR-ANO-2-2014-00352 and conducted an evaluation to verify the capability of the ventilation system. Failure to ensure that the ventilation system was capable of cooling the switchgear under design basis conditions was a performance deficiency. The performance deficiency was more than minor, and therefore a finding, because it was associated with the design control attribute of the Mitigating Systems Cornerstone and adversely affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Using Manual Chapter 0609, Attachment 4, Initial Characterization of Findings, June 19, 2012, and Appendix A, The Significance Determination Process (SDP) for Findings at Power, June 19, 2012, Exhibit 2, Mitigating Systems Screening Questions, the inspectors determined that the finding was of very low safety significance (Green) because the finding was a deficiency affecting the design of a mitigating system, and the system maintained its functionality. The inspector
determined that there was no cross-cutting aspect associated with this finding because the cause of the performance deficiency occurred more than three years ago, and was not representative of current licensee performance. |
