Docket No. 50-361 30-Day Report Licensee Event Report No.82-141 San Onofre Nuclear Generating Station, Unit 2 Pursuant to Section 6.9.1.13b of Appendix A, Technical Specifications to Operating License NPF-10, for San Onofre Unit 2, this submittal provides the required 30-day written report and copy of Licensee Event Report (LER) for an occurrence involving the Condensate Storage Tank T-120 low water level.

Limiting Condition for Operation (LC0) 3.7.1.3 requires the condensate storage tank T-120 to be operable in Modes 1, 2 and 3 with a contained water volume of at least 280,000 gallons. On October 27,1982 at 1700 while in Mode 2, the water level in Tank T-120 dropped below this required limit, the tank was declared inoperable, and the Action Statement associated with LC0 3.7.1.3 was entered. This Action Statement requires the inoperable tank to be returned to service within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> or cooldown must be initiated.

The cause of this incident was due to misalignment of valving for make up to tank T-120. This was due to failure to recognize and account for system configuration as modified by a properly documented temporary modification.

Engelken November 24, 1982 Corrective measures to restore water level were in progress prior to the level falling below the required limit, however, these measures were not sufficient to prevent the level from dropping below the limit for approximately 31/2 hours, at which time the level was restored to the required limit and the LC0 again satisfied.

Corrective actions to prevent recurrence include additional training of operators to ensure that the latest drawings and other required documents are used in any plant evolution. Also, an engineering evaluation to implement an automatic level control system is underway. The completion date for this evaluation has not been established.

The public health and safety were not affected since the tank was restored to operable status within the permisible time. Also, the operating history at this point in plant start-up precludes significant decay heat. Attached LER 82-141 addresses this event.
