Dear Mr. Engelken:

My letter dated November 3, 1981, forwarded to you our LER 81-024 concerning failure of Energy Products Group (EPG) ball valves with Efcomatic j

Actuators to trip initially. My followup letter of December 1,1981 provided further information relative to the implementation of our preventive maintenance and operability inspection program. This letter is written to report the failure of CV-526 to close on the first two closing attempts during the January operability test. CV-526, one of two containment isolation valves in the reactor coolant letdown line, was one of three ball valves reported in my November 3, 1981 letter that failed to operate on initial demand.

Licensee Event Report 82-002 is attached.

Additional information concerning this event report is provided below.

The operability testing program was instituted as a result of previous M

failures of valves with Efcomatic Actuators.

This program requires monthly 4} g cycling of selected valves. During the tests conducted on January 28, 1982, CV-526 did not close on the first two initiations of the remote manual close switch in the control room.

Prior to a third closing attempt, the operator pushed the open control switch.

He then pushed the close switch and the valve

g moved to its closed position.

The valve was reopened and was subsequently cycled two additional times.

The valve operated satisfactorily.

In accordance with the valve exercise procedure, minor maintenance was performed which included lubrication and inspection of the vahe.

No abnormalities were observed during the inspection.

. The event described above is consistent with the initial failures to operate described in my November 3, 1981 letter,.and, for the reasons stated therein, CV-526 was and is considered operable.

Furthermore, as reported in my December 1, 1981 letter, we have initiated a program of refurbishment and modification to improve the reliability and operability of all Efcomatic-actuators. Our present schedule indicates replacement of installed Efcomatic actuators with refurbished actuators during the March, 1982 outage.

Additionally, SCE has initiated long term action to replace these actuators with others of a more reliable design.
