Docket No. 50-361 30-Day Report Licensee Event Report No.82-126 San Onofre Nuclear Generating Station, Unit 2 Pursuant to Section 6.9.1.13b of Appendix A to Facility Operating License NPF-10 for San Onofre Unit 2, this submittal provides the required written report and copy of Licensee Event Report (LER) for an occurrence involving the containment isolation valves. A completed copy of LER 82-126 is enclosed.

On October 10, 1982, while in Mode 4 during routine Control Room in-spection, mainsteam trap isolation valve 2HV-8249 was observed to have lost position indication. The valve was declared inoperable at 0220 and Limiting Condition for Operation (LCO) 3.6.3 Action Stetement b was l

invoked. As required by this Action Statement, the penetration was isolated by de-energization of the valve and local verification of its closure.

Subsequent investigation revealed that the trouble was due to loosening of the magnetic reed switch assembly 2ZSL-8249-2 on the valve stem. The loosening of this switch assembly allowed it to rotate causing the close limit switch to open due to its misalignment with the magnet. This condition resulted in lost valve position indication in the Control Room. After properly aligning and tightening the switch, the valve was declared operable in accordance with 5023-3-3.30 at 2140 on October 10, 1982. As corrective action, Electrical Maintenance has been instructed to examine all valves of this type to verify proper reed switch alignment and tightness.
