No. 14 Steam Generator Level Channel 3 - Inoperable.

This report was initiated by Incident Report 81-453.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 80% - Unit Load 970 MWe DESCRIPTION OF OCCURRENCE:

On November 8, 1981, during normal operation, the No. 14 Steam Generator Level Channel 3 failed high.

Channel 3 was declared inoperable and Action Statement 3.3.2.1 Action 14 was entered at 1942 hours0.0225 days <br />0.539 hours <br />0.00321 weeks <br />7.38931e-4 months <br />.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the inoperable channel was the failure of the dual comparator.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.2.1 Action 14 requires:

With one inoperable channel, operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

CORRECTIVE ACTION

The dual comparator was replaced, and a channel functional test was satisfactorily performed.

The channel was declared operable, placed in service and Action Statement 3.3.2.1 Action 14 was terminated at 2210 hours0.0256 days <br />0.614 hours <br />0.00365 weeks <br />8.40905e-4 months <br />.
