Auxiliary Feed Storage Tank - Low Level This report was initiated by Incident Report 81-448.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

On November 6, 1981 during recovery actions from a reactor trip, the Auxiliary Feed Storage Tank (AFST) was observed to be below the minimum level required by the technical specifications.

At 1353 hours0.0157 days <br />0.376 hours <br />0.00224 weeks <br />5.148165e-4 months <br />, the AFST was declared inoperable and Action Statement 3.7.1.3.a was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the low level in the AFST was the feeding of the steam generators with auxiliary feedwater following a reactor trip at 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br />.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.1.3.a requires:

With the Auxiliary Feed Storage Tank inoperable, within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> restore to operable or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION

Demineralized water makeup was initiated and the proper level was restored in the AFST.

The AFST was declared operable and Action Statement 3.7.1.3.a was terminated at 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br />.
