Departure from Nucleate Boiling Parameters - Low Pressurizer Pressure.

This report was initiated by Incident Report 81-417.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 95% - Unit Load 1040 MWe DESCRIPTION OF OCCURRENCE:

On October 21, 1981, while cycling Pressurizer Auxiliary Spray Line Isolation Valve 1CV75 during surveillance, the pressure in the pressurizer dropped below 2205 PSIA for a total of five minutes.

Action Statement 3.2.5.b was entered at 1820 hours0.0211 days <br />0.506 hours <br />0.00301 weeks <br />6.9251e-4 months <br />.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The loss of pressurizer pressure was due to incorrect valve liftoff pressure setting requiring excessive time to cycle the 1CV75 air-operated valve open and closed, allowing an excessive amount of makeup water to condense the, team bubble in the pressurizer.

CORRECTIVE ACTION

The 1CV75 valve was closed by the Control Operator and the pressurizer pressure returned to normal.

Action Statement 3.2.5.b was terminated at 1825 hours0.0211 days <br />0.507 hours <br />0.00302 weeks <br />6.944125e-4 months <br />, October 21, 1981.

It was subsequently discovered that the valve liftoff pressure setting provided by the manufacturer was incorrect at 11 PSI.

The liftoff pressure was adjusted to 23 PSI and the valve was tested satisfactorily.
