Auxiliary Feed Storage Tank - Below Minimum Volume.

This report was initiated by Incident Reports81-411 and 81-435.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

On two occasions, October 17 and October 29, 1981, each following a reactor trip, the Auxiliary Feed Storage Tank dropped below the minimum required volume of 200,000 gallons due to steam generator makeup.

At 1456 and 1358 hours0.0157 days <br />0.377 hours <br />0.00225 weeks <br />5.16719e-4 months <br />, on October 17 and 29, respectively, the Auxiliary Feed Storage Tank was declared inoperable, and Action Statement 3.7.1.3.a was entered.

These occurrences constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of the drop in Auxiliary Feed Storage Tank volume below the required minimum, on both occasions, was steam generator makeup following a reactor trip.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.1.3.a requires:

With the Auxiliary Feed Storage Tank inoperable, within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> either restore the AFST to operable status or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION

Domineralized water was transferred to the Auxiliary Feed Storage Tank and the volume was restored to greater than the minimum specification of 200,000 gallons.

At 1845 and 1617 hours0.0187 days <br />0.449 hours <br />0.00267 weeks <br />6.152685e-4 months <br />, respectively, the Auxiliary Feed Storage Tank was declared operable and Action Statement 3.7.1.3.a was terminated.
