DESCRIPTION OF OCCURRENCE:

On September 22, 1981 an operator noticed a so.tvice water leak

emanating from a crack in a dissimilar metal weld on drain valve 12SW124 between the valve and No. 12 Component Cooling

Water Heat Exchanger.

Service water.was isolated and No. 12 Component Cooling Water Heat Exchanger was declared inoperable.

q This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCES l

1 The dissimilar metal weld between the valve body and caibon

steel pipe corroded and cracked.

ANALYSIS OF OCCURRENCE:

Technical specification 3.7.3.1 requires:
