IDENTIFICATION OF OCCURRENCE:

No. 12 Charging Pump - Inoperable.

This report was initiated by incident report 81-358.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx power 93% - Unit Load 960 MWe DESCRIPTION OF OCCURRENCE:

On September 2, 1981 an operator noticed service water emanating from a leak in the oil cooler of No. 12 charging pump.

At 1230 hours0.0142 days <br />0.342 hours <br />0.00203 weeks <br />4.68015e-4 months <br /> No. 12 charging pump was declared inoperable.

Since No. 13 charging pump was also inoperable, action statements 3.1.2.4 and 3.5.2.a were entered.

This occurrence constituted operation in a degraded mode in accordance with technical specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

l The cause of the service water leak was leaks in two couplings in the service water piping.

ANALYSIS OF OCCURRENCE:

Technical specification 3.1.2.4.

CORRECTIVE ACTION

The service water leaks were repaired by replacing the two leaking service water couplings.

At 2035 hours0.0236 days <br />0.565 hours <br />0.00336 weeks <br />7.743175e-4 months <br />, September 2, 1981, No. 12 charging pump was tested satisfactorily end declared operable.

Action statements 3.1.2.4 and 3.5.2.a were terminated.
