Missed Surveillance - Triaxial Time-History Accelographs This report was initiated by Incident Report 81-340.

CONDITIONS PRIOR TO OCCURRENCE:

On August 24, 1981, it was noticed that the surveillance procedure 13.2.003 for channel check on the triaxial time-history accelographs, due on July 12, 1981, had been overlooked.

The surveillance frequency I

is once per month in accordance with surveillance requirement 4.3.3.3.1.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The reaaen the surveillance was missed was because the inspection order (10) card had been misplaced.

The omission was discovered on the overdue list.

ANALYSIS OF OCCURRENCE:

Surveillance requirement 4.3.3.3.1 requires:

Each of the seismic monitoring instruments shall be demonstrated operable by the performance of the channel check, channel calibration, and channel i

functional test operations at the frequencies shown in table 4.3-4.

CORRECTIVE ACTION

Surveillance procedure 13.2.003 was performed satisfactorily immediately upon the discovery of the omission.

The personnel responsible for those functions were counseled concerning their frequency and importance.
