Peport Number: 81-75/03L Report Date: 8-28-81 Occurrence Date: 8-9-81 Facility: Salem Generating Station, Unit 1 Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE: Containment Fan Coil Unit - Inoperable - No Service Water Flow Indication. This report was initiated by Incident Report 81-320. CONDITIONS PRIOR TO OCCURRENCE: Moda 1 - Rx Power 71% - Unit Load 730 MWe DESCRIPTION OF OCCURRENCE: On August 9, 1981, the control roo.n operator noticed that there was no service water flow indication on No. 13 containment fan coil unit (CFCU). The CFCU was declared. inoperable and action statement 3.6.2.3.a was entered at 0530 hours, August 9, 1981. This occurrance constituted operation in a degraded mode in accord-ance with technical specification 6.9.1.9.b. DESIGNATION OF ADPARENT CAUSE OF OCCURRENCE: Equipment failure. The low pressure side of flow transmitter PA-3543Z-1 was plugged with silt. ANALYSIS OF OCCURRENCE: Technical specification 3.6.2.3.a requires with: One group of containment cooling fans inoperable and both Containment Spray Systems operable, restore the inoperable group of cooling fr.ns to operable status within 7 days or be in at least hot standby within the next 6 hours and in cold shutdown within the following 30 hours.

CORRECTIVE ACTION

Flow transmitter PA-3543Z-1 sensing line was blown clear and the transmitter was recalibrated satisfactorily. The No. 13 CFCU was tested satisfactorily and returned to service. Action statement 3.6.2.3.a was terminated at 1137 hours on August 9, 1981. A program is currently in effect to blow down all transmitters at regular intervals.
